摘  要:目的基于血氧水平依赖(blood oxygenation level dependent,BOLD)技术的功能性磁共振成像(functional magneticresonance imaging,fMRI)分析脑卒中后运动性失语症(BrocaAphasia,BA)患者动词产生任务下脑区的激活特征,初步探讨脑卒中后运动性失语症的动词产生机制。方法选择符合纳入和排除标准的BA患者31例(失语症组),采用失语症检查法进行语言功能评估,以名词为基本实验材料,在动词产生任务下行BOLD-fMRI,并扫描与失语症组相匹配的36例正常志愿者(正常组)。采用SPM8.0软件进行图像预处理及统计分析,并比较正常组和失语症组的脑激活差异区。结果正常组在左侧额中回、左侧缘上回、左侧梭状回、左侧舌回、左侧额上回、脑干和右侧中央前回有显著激活,且激活强度显著大于失语症组(P<0.05);失语症组在左侧颞横回有显著激活,且激活强度大于正常组(P<0.05)。结论BA患者可能存在动-名词分离现象;左侧额中回可能不仅参与语音、语义分析,而且在动词加工中起重要作用。Objective Based on the blood oxygen level dependent(blood oxygenation level dependent,BOLD)techniques of functional magnetic resonance imaging(functional magnetic resonance imaging,fMRI)to study activation character of Broca Aphasia(BA)patients’after stroke under language tasks,and preliminary discuss the difference of brain activation areas between patients and the healthy.Methods Select 31 cases of patients(patient group)with BA meet inclusion and exclusion criteria,adopt the aphasia test to assess language function,and use nouns as the basic experimental material,conduct BOLD-fMRI under the verb produce task.Scan 36 cases of healthy volunteers(healthy group)matched with the patient group.Use SPM8 software to conduct image preprocessing and statistical analysis,and compare healthy and aphasia groupof brain activation difference areas.Results The healthy group showed larger activation in left middle frontal gyrus,left supramarginal gyrus,left fusiform gyrus,left lingual gyrus,left frontal gyrus,brain stem,and right precentral gyrus(P<0.05);Aphasia group shows larger activation in left temporal transverse gyrus(P<0.05).Conclusion BA patients may be exist noun-verb separation phenomenon;The left middle frontal gyrus may not only participate in the phonetic,semantic analysis,but also may be play an important role in the verb processing.
