改良鼻中隔缝合技术在鼻中隔偏曲矫正术联合下鼻甲等离子射频消融术后的应用效果

摘　　要：目的讨论改良鼻中隔缝合技术在鼻中隔偏曲矫正术联合下鼻甲等离子射频消融术后的应用效果。方法选取2017年1月至2018年1月本院耳鼻喉科收治的46例鼻中隔偏曲并伴有下鼻甲肥大(单纯下鼻甲肥大、血管运动性鼻炎、过敏性鼻炎、慢性肥厚性鼻炎)患者为研究对象,根据患者住院号奇偶数随机分为缝合组和填塞组,每组23例。两组患者均在鼻内镜下行粘软骨膜下鼻中隔偏曲矫正术和等离子射频下鼻甲消融术。术后缝合组改良缝合鼻中隔黏膜,而填塞组应用膨胀海绵填塞双侧鼻腔。比较术后两组临床症状及术后48 h及术后1个月并发症情况。结果缝合组术后鼻塞、头痛、溢泪、耳闷、睡眠困难及进食困难等VAS评分均低于鼻腔填塞组,差异有统计学意义(P<0.05)。两组术后48 h及术后1个月内鼻出血、鼻中隔血肿、鼻中隔穿孔、感染发生率比较差异无统计学意义。结论针对鼻中隔偏伴有下鼻甲肥大的患者,改良鼻中隔缝合技术仍然是较好的选择。Objective To discuss the application of improved nasal septum suture technique after nasal septum deviation correction combined with inferior turbinate plasma radiofrequency ablation.Methods 46 cases patients with rhinitis of nasal septum deviation and inferior turbinate hypertrophy(simple inferior turbinate hypertrophy,vasomotor rhinitis,allergic rhinitis,and chronic hypertrophy)admitted to the Department of Otolaryngology in our hospital from January 2017 to January 2018 were selected as the study subjects.They were randomly divided into two groups based on the odds and even numbers of the hospitalization number at the time of hospitalization,namely the suturing group and the stuffing group,with 23 cases in each group.Inferior nasal septum deviation correction and plasma radiofrequency inferior turbinate ablation.The postoperative suturing group modified sutured nasal septum mucosa,while the stuffing group used expansion sponge to fill both sides of the nasal cavity.The clinical symptoms and complications at 48 h and 1 month after operation were compared between the two groups.Results The VAS scores of postoperative nasal congestion,headache,tears,ear tightness,difficulty sleeping,and eating difficulties were lower in the suturing group than in the nasal stuffing group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of bleeding,nasal septum hematoma,nasal septum perforation,infection and infection within one month after surgery.Conclusion For patients with deviated nasal septum accompanied by inferior turbinate hypertrophy,improved nasal septum suture technique is still a better choice.
