不典型与典型肺结核抗酸杆菌、CA125、CEA检测结果对比分析

摘　　要：目的探讨不典型与典型肺结核抗酸杆菌、CA125、CEA检测结果,提升对该类患者的临床认知,提高诊断的准确性。方法选取超声雾化导痰行痰培养、肺活检确诊为不典型症状肺结核患者60例为不典型组,选取同期有典型症状肺结核患者60例为典型组,比较2组患者的一般情况、影像学资料和实验室检查数据差异情况。结果无典型症状组痰结核菌检查阳性率为31.7%(19例),有典型症状组痰结核菌检查阳性率83.3%(50例),2组差异具有统计学意义(P<0.05);2组患者AFB检查的阳性率各级别组间差异无统计学意义(P>0.05);无典型症状组患者血清中CA125水平低于典型症状组患者水平,差异具有统计学意义(P<0.05),无典型症状组患者血清中CEA略低于典型症状组,差异无统计学意义(P>0.05);CT检查可从叶、段实变型结节肿块,肺门、纵膈淋巴结肿大等均有其特性。结论无典型症状肺结核相较典型症状老年肺结核患者肺脏组织损伤较轻,CT影像资料有明显差异。Objective To explore the results of atypical and typical tuberculosis acid-fast bacilli,CA125,CEA,improve the clinical cognition of such patients,and improve the accuracy of diagnosis.Methods A total of 60 patients with atypical pulmonary tuberculosis diagnosed by ultrasonic nebulization sputum culture and lung biopsy were selected as the atypical group,and 60 patients with typical symptoms in the same period were selected as the typical group.The general situation,imaging data and laboratory examination data of the two groups were compared.Results The positive rate of tuberculosis test was 31.7%(19 cases)in the atypical symptom group,and 83.3%(50 cases)in the typical symptom group.The difference between the two groups was statistically significant(P<0.05).There was no significant difference in the positive rate of AFB between the two groups(P>0.05).The serum CA125 level in patients without typical symptoms was lower than that in the typical symptom group(P<0.05).The serum CEA in patients without typical symptom group was slightly lower than that in the typical symptom group,the difference was not statistically significant(P>0.05);CT examination can be performed from the leaf,segmental consolidation nodular mass,hilar,mediastinal lymphadenopathy,etc.Conclusion There is no typical symptom of pulmonary tuberculosis compared with typical symptoms.The lung tissue damage of elderly pulmonary tuberculosis patients is lighter,and CT imaging data are significantly different.
