摘　　要：目的探讨奥马哈系统联合推拿对维持性血透(MHD)患者的影响。方法选取2016年5月—2018年7月在我院血液净化中心经MHD的患者94例,随机分为常规组和研究组,各47例。其中常规组采用常规治疗+常规护理干预;研究组在常规组的基础上实施奥马哈系统个案管理联合推拿疗法。观察2组血浆心钠素(ANP)、低密度脂蛋白胆固醇(LDL-C)情况,透析充分性,口渴程度、血压、水肿情况以及中文版Piper疲乏评估量表(RPFS-CV)评分。结果干预后,2组ANP、LDL-C、口渴程度、收缩压、舒张压、水肿情况、RPFS-CV总分均降低,且研究组低于常规组(P<0.05);而2组尿素下降率(URR)、尿素清除指数(KT/V)均升高,且研究组高于常规组(P<0.05)。结论奥马哈系统联合推拿可降低MHD患者ANP、LDL-C水平,改善透析充分性,并能改善临床症状及疲乏症状,进而有助于提高生活质量。Objective To investigate the effect of Omaha system combined with massage in maintenance hemodialysis(MHD)patients.Methods A total of 94 patients with MHD patients in our blood purification center between May 2016 to July 2018 were selected.They were randomly divided into the regular group(47 cases)and the research group(47 cases).The regular group received regular treatment+routine nursing intervention.The research group implemented Omaha system case management combined massage therapy on the basis of the regular group.The atrial natriuretic peptide(ANP)and low-density lipoprotein cholesterol(LDL-C)status,dialysis adequacy,thirst,blood pressure,edema and the score of the revised piper fatigue scale-chinese version(RPFS-CV)were compared.Results After treatment,the levels of ANP,LDL-C,the thirst,systolic blood pressure,diastolic blood pressure and edema,and the total scores of RPFS-CV in both groups decreased,and the research group were lower than the regular group(P<0.05);while the URR and KT/V in the two groups increased,and the research group were higher than the regular group(P<0.05).Conclusion Omaha system combined with massage could reduce ANP and LDL-C levels in MHD patients,improve dialysis adequacy,and improve clinical symptoms and fatigue symptoms,thereby helping to improve the quality of life.
