摘  要:目的探讨复合乳酸菌胶囊对小儿慢性腹泻病的疗效及对其肠道微生态的影响。方法选取2017年1月至2018年10月湖北省鄂州市中心医院收治的82例患有慢性腹泻病的小儿,采用随机数字表法将其分为对照组与观察组,每组各41例,对照组给予口服补液盐、纠正水电酸碱平衡、抗感染治疗等常规治疗,观察组在对照组基础上给予复合乳酸菌胶囊治疗。比较两组治疗后的止泻时间及排便频率;比较两组小儿慢性腹泻病的临床疗效;比较两组治疗后的肠道微生态结构及复杂度;比较两组不良反应发生情况;结果治疗后观察组的止泻时间[(2.24±0.45)d]显著短于对照组[(3.56±0.71)d](P<0.05),排便频率[(1.59±0.32)次/日]显著低于对照组[(5.18±1.04)次/日](P<0.05);两组小儿慢性腹泻病的临床疗效等级分布差异有统计学意义(P<0.05),观察组总有效率显著高于对照组(P<0.05);治疗后两组小儿肠道微生物门水平组成基本一致,但不同菌门比例差异显著(P<0.05),且肠道中优势菌属丰度差异显著(P<0.05),其中,观察组的双歧杆菌属[(9.98±1.99)lg拷贝数/g湿便]、乳杆菌属[(8.68±1.74)lg拷贝数/g湿便]显著高于对照组[(8.26±1.65)lg拷贝数/g湿便、(7.34±1.47)lg拷贝数/g湿便](P<0.05),大肠杆菌属[(6.12±1.22)lg拷贝数/g湿便]、肠球菌属[(6.43±1.29)lg拷贝数/g湿便]显著低于对照组[(7.86±1.57)lg拷贝数/g湿便、(7.86±1.62)lg拷贝数/g湿便](P<0.05);观察组治疗后肠道微生态菌群丰富度(Chao1)指数[(5.32±1.06)]及菌群多样性香农(Shannon)指数[(3.17±0.63)]均显著高于对照组[(2.68±0.54)、(2.34±0.47)](P<0.05);两组不良反应发生率差异无统计学意义(P>0.05)。结论复合乳酸菌胶囊治疗小儿慢性腹泻病可有效缩短小儿止泻时间,降低排便频率,增强疗效,改善肠道微生态结构及复杂度,且不会增加不良反应发生率。Objective To investigate the effect of compound lactobacillus capsule on chronic diarrhea in children and its effect on intestinal microecology.Methods A total of 82 children with chronic diarrhea admitted to the Ezhou Central Hospital of Hubei from January 2017 to October 2018 were selected and randomly divided into control group and observation group by random number table method,with 41 children in each group.The control group received routine treatment,and the observation group received complex lactobacillus capsule treatment on the basis of the control group.The antidiarrheal time,frequency of defecation after treatment,the clinical effect of chronic diarrhea in children,the changes of intestinal microecology after treatmentand the adverse reactions in the two groups were compared.Results The antidiarrheal time of the observation group[(2.24±0.45)d]was significantly shorter than that of the control group[(3.56±0.71)d](P<0.05),and the frequency of defecation after treatment of the observation group[(1.59±0.32)times/d]was significantly lower than that of the control group[(5.18±1.04)times/d](P<0.05).The clinical efficacy grade distribution of chronic diarrhea in children in the two groups was significantly different(P<0.05),and the total effective rate in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the composition of intestinal microflora in the two groups was basically the same,but the proportion of different microflora was significantly different(P<0.05),and the abundance of dominant bacteria in the intestine was significantly different(P<0.05).The bifidobacterium[(9.98±1.99)lg copy/g wet stool]and lactobacillus[(8.68±1.74)lg copy/g wet stool]of the observation group were significantly higher than those of the control group[(8.26±1.65)lg copy/g wet stool,(7.34±1.47)lg copy/g wet stool](P<0.05),while escherichia coli[(6.12±1.22)lg copy/g wet stool],enterococcus[(6.43±1.29)lg copy/g wet stool]of the observation group were significantly lower t
