摘 要:目的探究肾脏移植术后继发性感染的病原菌分布、药敏性。方法选取2017年3月-2019年5月在本院治疗的肾移植患者178例,均采集其术后痰液、腹腔引流液、血液、尿液、伤口分泌物等各种标本,进行病原菌培养及药敏试验,分析其病原菌分布及药敏性。结果铜绿假单胞菌对氨苄西林/舒巴坦的耐药率较高,为90.01%;肺炎克雷伯菌对头孢呋辛钠、头孢唑林、阿莫西林/克拉维酸的耐药率较高,为83.33%,对美罗培南的耐药率为0.00%;大肠埃希菌对阿莫西林/克拉维酸、左旋氧氟沙星的耐药率较高,分别为100.00%、80.00%,对舒普深的耐药率较低,为20.00%;金黄色葡萄球菌对左旋氧氟沙星的耐药率较高,为70.00%,对替考拉宁、万古霉素、舒普深的耐药率均为0.00%。结论肾脏移植术后继发性感染发生率较高,且以革兰阴性菌、革兰阳性菌为主,且对抗生素均存在一定的耐药性,需加强耐药监测,做好防治工作。Objective To investigate the pathogenic bacteria distribution and drug sensitivity of secondary infections after kidney transplantation. Methods A total of 178 patients with chronic renal failure who underwent renal transplantation in our hospital from March 2017 to May 2019 were enrolled in this study. All the specimens inclusing postoperative sputum,abdominal drainage fluid,blood,urine,wound secretion were collected for pathogen culture and drug sensitivity test,so as to analyze the distribution of pathogens and drug susceptibility. Results The resistance of Pseudomonas aeruginosa to amoxicillin/sulbactam was high( 90. 01%);Klebsiella pneumoniae has higher resistance to cefuroxime sodium,cefazolin,amoxicillin/clavulanic acid,accounting for 83. 33%,and low resistance to meropenem,accounting for 0. 00%;Escherichia coli has high resistance to amoxicillin/clavulanic acid and levofloxacin,accounting for 100. 00% and 80. 00%,respectively,and low resistance to sulperazone,accounting for 20. 00%;Staphylococcus aureus has a high resistance to levofloxacin,accounting for 70. 00%,and a low resistance to teicoplanin,vancomycin,and sulperazone,accounting for 0. 00%. Conclusion The incidence of secondary infection after renal transplantation is high,and Gram-negative bacteria and Gram-positive bacteria are the main ones,and there is certain resistance to antibiotics. It is necessary to strengthen drug resistance monitoring and do a good job in prevention and treatment.
