摘 要:目的探讨脐血IRAK-M基因多态性与早产的关系。方法收集2015年1月-2018年9月于本院出生的60例早产儿和60例健康足月儿的脐血,检测IRAK-M基因+22148G> A位点。ELISA法检测2组细胞炎性因子浓度(IL-1β、IL-6、IL-10、TNF-α)。结果早产儿组与对照组G/G基因型频率分别为33.3%和20.0%(χ2=4.269,P> 0.05),G等位基因频率分别为51.7%和36.7%(P> 0.05)。合并胎膜早破及绒毛膜羊膜炎的早产儿,其G/G基因型及G等位基因频率分别为63.6%、79.5%,明显高于对照组(P <0.05)。早产儿组IL-1β、IL-6、TNF-α浓度明显高于对照组(P <0.05)。早产儿组IL-10浓度与对照组比较差异无统计学意义(P> 0.05)。结论 G/G基因型新生儿更倾向于发生早产,该基因型与合并胎膜早破及绒毛膜羊膜炎的早产密切相关。Objective To determine the correlation between preterm birth and the polymorphism of IRAK-M in umbilical cord blood. Methods Umbilical cord blood was collected from 60 premature infants and 60 healthy full-term infants born in our hospital from January 2015 to September 2018,and IRAK-M gene + 22148 G > A locus was detected. The concentrations of inflammatory cytokines( IL-1β,IL-6,IL-10,TNF-α) were measured by ELISA in both groups. Results In preterm group and control group,the frequency of G/G type was 33. 3% and 20. 0%( χ^2= 4. 269,P > 0. 05) and that of G allele was51. 7% and 36. 7%( P > 0. 05),respectively. The G/G genotype and G allele frequency of the premature infants with premature rupture of membranes and chorionic amnitis were 63. 6% and 79. 5%,respectively,which were significantly higher than those of the control group( P < 0. 05). The concentrations of IL-1β,IL-6 and TNF-α in the premature infant group were significantly higher than those in the control group( P < 0. 05). The concentration of IL-10 in the premature infant group was higher than that in the control group,with the differences statistically significant( P > 0. 05). Conclusion Newborns with G/G genotype are more prone to preterm birth,and this genotype is closely associated with preterm birth with premature rupture of membranes and chorionic amnitis.
