摘 要:目的研究高效抗反转录病毒治疗(highly active anti-retroviral therapy,HARRT)过程中HIV(human immunodeficiency virus,HIV-1)感染者外周血miR-155表达变化,探讨miR-155的临床价值。方法招募53例接受HAART的HIV-1感染者和50例健康对照者,运用RT-PCR法检测HARRT治疗0个月、1个月、3个月、6个月及12个月感染患者外周血miR-155表达水平。结果接受HAART治疗组CD8+T细胞miR-155相对表达水平显著高于健康对照组(t=5.559,P=0.000)。HARRT治疗过程中,HIV-1感染者CD8+T细胞miR-155表达水平逐渐下降,治疗前、治疗1个月、治疗3个月、治疗6个月及治疗12个月相对表达水平分别为(391.47±100.58)%、(373.92±101.60)%、(343.24±100.73)%、(299.39±93.08)%和(184.67±124.62)%,各治疗节点差异有统计学意义(F=35.732,P=0.000)。HIV感染者CD8^+T细胞miR-155表达水平与HIV-RNA、CD8^+CD38^+T细胞呈正相关(r值分别为0.578和0.663,P=0.000)。结论 HIV-1感染者外周血CD8^+T细胞中miR-155水平显著升高,HAART治疗能够降低其表达水平,可作为监测治疗疗效的指标。Objective To study the changes of miR-155 expression in peripheral blood of HIV patients with highly active anti-retrovirus therapy,and evaluate the clinical value of miR-155. Methods Fifty-three HIV-1 patients with highly active antiretroviral therapy( HAART) and 50 controls were recruited into this study. MiR-155 levels in CD8^+T cells were detected by quantitative reverse transcription( RT-PCR) at 0,1,3,6 and 12 month after treatment with HARRT.Results The level of miR-155 in patients with HARRT is higher than that in the controls( t = 5. 559,P = 0. 000). During the course of HARRT treatment,miR-155 level of HIV patients were gradually decreased,and the levels before treatment,1 month after treatment,3 months after treatment,6 months after treatment and 12 months after treatment were( 391. 47 ±100. 58) %,( 373. 92 ± 101. 60) %,( 343. 24 ± 100. 73) %,( 299. 39 ± 93. 08) % and( 184. 67 ± 124. 62) %,respectively.There was statistical significance on the differences in each period( F = 35. 732,P = 0. 000). MiR-155 levels were positively correlated with HIV-RNA and CD8^+CD38^+T cell( r = 0. 578 and 0. 663,P = 0. 000). Conclusion The level of miR-155 in CD8^+T cells in peripheral blood of HIV-1 infected patients was significantly increased,and HAART treatment could reduce its level,which could be used as an indicator to monitor the therapeutic effect.
