神经内镜辅助下小骨窗入路手术治疗分隔型慢性硬膜下血肿的临床分析

摘　　要：目的探讨分隔型慢性硬膜下血肿(CSDH)患者采用神经内镜辅助下小骨窗入路手术的治疗效果。方法采用随机数字表法将我院2018年1月—2019年4月收治的60例分隔型CSDH患者分为2组,各30例。对照组行钻孔闭式引流术,观察组行神经内镜辅助下小骨窗入路手术,比较2组临床指标及术后随访情况。结果观察组住院时间及引流时间比对照组短,手术时间比对照组长;观察组术后7 d时血肿消失率高于对照组,观察组术后1年时血肿复发率低于对照组,差异均有统计学意义(P<0.05)。观察组术后7 d时分隔腔残存率、术后1年时病死率略低于对照组,但差异无统计学意义(P>0.05)。结论分隔型CSDH患者采用神经内镜辅助下小骨窗入路手术的效果较好,可有效清除血肿,减少复发,改善预后。Objective To explore the effect of small endoscopy-assisted small bone window approach for patients with separated chronic subdural hematoma(CSDH).Methods A random number table method was used to divide 60 cases of separated CSDH patients admitted to our hospital from January 2018 to April 2019 into 2 groups of 30 patients.The control group underwent drilling and closed drainage,and the observation group underwent neuroendoscopy-assisted small bone window approach surgery to compare the clinical indicators and postoperative follow-up of the two groups.Results The hospitalization time and drainage time of the observation group were shorter than that of the control group,and the operation time was longer than that of the control group.The disappearance rate of hematoma was higher than that of the control group at 7 days after operation.All of them were statistically significant(P<0.05);the survival rate of the separation chamber at 7 days after operation and the mortality rate at 1 year after operation were slightly lower than those of the control group,but the difference was not statistically significant(P>0.05).Conclusion The neuroendoscopy-assisted small bone window approach is effective in patients with separated CSDH.It can effectively remove hematoma,reduce recurrence,and improve patient prognosis.
