摘 要:目的探讨与分析血清中信号转导子和转录激活子(signal transducers and activators of transcription,STAT)-6、半乳糖凝集素(Galectin)-9、胸腺基质淋巴细胞生成素(Thymic stromal lymphopoietin,TSLP)、微小RNA(miR)-31表达及炎症因子的变化在哮喘儿童中的诊断价值。方法采用随机对照研究,研究时间为2016年2月到2018年1月,选择在北京市大兴区人民医院诊治的哮喘患儿72例作为观察组,同期选择健康儿童72例作为对照组,检测两组血清中STAT-6、Galectin-9、TSLP、miR-31、炎症因子与全血miR-31含量,并进行相关性分析。结果观察组血清TNF-ɑ、IL-6、STAT-6、Galectin-9、TSLP含量和全血miR-31相对表达水平显著高于对照组(P<0.05)。治疗后肺功能指标FEV 1%和PEF%均高于治疗前,炎症因子及STAT-6、Galectin-9、TSLP、miR-31表达均低于治疗前(P<0.05)。在观察组患中,Pearson相关性分析显示血清TNF-ɑ、IL-6、STAT-6、miR-31、TSLP、miR-31含量与肺功能指标FEV 1%和PEF%呈显著负相关性,与哮喘病情等级正相关(P<0.05)。ROC曲线显示炎症因子TNF-α、IL-6/STAT-6、Galectin-9、TSLP、miR-31曲线下面积分别为0.922,0.947,0.918,0.927,0.936,0.937,炎症因子和STAT-6、Galectin-9、TSLP、miR-31检测儿童哮喘具很高的判断价值。结论哮喘患儿表现为血清STAT-6、Galectin-9、TSLP、miR-31含量显著升高,并伴随有炎症因子TNF-ɑ、IL-6的释放,与哮喘病情存在正相关性,可影响哮喘的发生。Objective To investigate and analyze the expression of serum signal transducers and activators of transcription(STAT)-6,galectin-9,thymic stromal lymphopoietin(TSLP),and miR-31 and the diagnostic value of changes in inflammatory factors for asthmatic children.Methods It used randomized controlled study.From February 2016 to January 2018,72 children with asthma who were diagnosed and treated at Beijing Daxing District People′s Hospital were selected as the observation group,and 72 healthy children were selected as the control group.The level of STAT-6,Galectin-9 and TSLP inflammatory factors in serum and miR-31 in the whole blood in the two groups were detected and given correlation analysis.Results The levels of TNF-α,IL-6 and other inflammatory factors in the observation group were significantly higher than those in the control group(P<0.05).The contents of serum STAT-6,galectin-9,TSLP and miR-31 in the observation group were significantly higher than those in the control group(P<0.05).The relative expression level of miR-31 in the observation group and the whole blood was significantly higher than that in the control group(P<0.05).After treatment,FEV 1%and PEF%were higher than those before treatment,and the expression of inflammatory factors,STAT-6,galectin-9,TSLP and miR-31 were lower than those before treatment(P<0.05).In the observation group,Pearson correlation analysis showed that serum TNF-λ,IL-6 and serum,STAT-6,miR-31,TSLP,miR-31 were negatively correlated with pulmonary function index FEV 1%and PEF%,and positively correlated with asthma classification(P<0.05).ROC curve showed that the area under the curve of TNF-α,IL-6/STAT-6,galectin-9,TSLP and miR-31 was 0.922,0.947,0.918,0.927,0.936,and 0.937,respectively.The detection of inflammatory factors and STAT-6,galectin-9,TSLP and miR-31 was of great value in the diagnosis of children with asthma.Conclusion The levels of serum STAT-6,Galectin-9 and TSLP are significantly increased in children with asthma,accompanied by the release of inflammatory facto
