应用儿童药物利用指数评价氟康唑在儿童用药中的合理性

摘　　要：目的:建立儿童药物利用评价方法,应用儿童药物利用指数(children drug utilization index,cDUI)评价氟康唑在儿童用药中剂量的合理性。方法:对河北省儿童医院2014年1月至2019年6月使用氟康唑的出院儿童病例进行回顾性分析,分别按照体质量与年龄进行分组,计算cDUI。结果:按体质量分组计算,使用氟康唑氯化钠注射液的患儿中,体质量<10 kg组的cDUI为0.82,表明用药剂量偏小;体质量20~30 kg组的cDUI为1.32,表明用药剂量偏大。使用氟康唑胶囊的患儿中,体质量20~30 kg组和>30 kg组的cDUI分别为0.80和0.81,表明用药剂量偏小。按年龄分组,使用氟康唑氯化钠注射液的患儿中,4~8岁组和8~15岁组的cDUI分别为1.30和1.64,表明用药剂量偏大。使用氟康唑胶囊的患儿中,8~15岁组cDUI为1.42,表明用药剂量偏大。结论:cDUI在评估儿童用药剂量合理性方面有一定可行性,但现有的评价方法仍存在一些缺陷,需进一步改进和优化。Objective:To establish an evaluation method for children’s drug use and to evaluate the rationality of fluconazole dosage in children by children drug utilization index(cDUI).Methods:The discharged child patients treated with fluconazole from January 2014 to June 2019 in the hospital were analyzed retrospectively.The subjects were divided in accordance with body weight and age,and related cDUIs were calculated.Results:With respect to body weight in the child patients treated with fluconazole and sodium chloride injection,the cDUI of the patients with a body weight in the<10 kg group was 0.82,indicating that the dosage was too low.On the other hand,the cDUI of those with a body weight in the 20-30 kg group was 1.32,indicating that the dosage might be too high.The cDUI of those with a body weight of 20-30 kg and>30 kg and treated with fluconazole capsules were respectively 0.80 and 0.81,which showed that the dosage might be too low.With respect to age,the cDUIs of the patients treated with fluconazole and sodium chloride injection in the 4-8 year and 8-15 year groups were respectively 1.30 and 1.64,indicating that the dosage might be too high.The cDUI of the patients treated with fluconazole capsules in the 8-15 year group was 1.42,which indicated that the dosage might be also too high.Conclusion:cDUI has certain feasibility in evaluating the rationality of drug dosage in children,nevertheless,existing evaluation methods still have certain shortcomings,which demands further improvement and optimization.
