呼吸湿化治疗仪在慢性阻塞性肺疾病致呼吸衰竭序贯通气中的应用

摘　　要：目的探讨呼吸湿化治疗仪在慢性阻塞性肺疾病(COPD)致呼吸衰竭序贯通气中的应用。方法选择我院ICU收治的40例COPD致呼吸衰竭的患者,随机分为观察组(20例)和对照组(20例),观察组采用呼吸湿化治疗仪进行序贯通气治疗,对照组采用传统双回路氧疗。比较两组患者脱机前和脱机后24h生命体征、血气分析、呼吸机相关性肺炎(VAP)发生率、再次机械通气率、ICU住院时间及住院费用。结果与对照组比较,脱机24h后观察组的呼吸频率(RR)、动脉血氧分压(PaO_2)、氧合指数(PaO_2/FiO_2)改善更明显(P<0.05),且观察组的机械通气时间及住院时间更短,住院费用更低。结论呼吸湿化治疗仪能改善患者的症状,减少住院费用,值得临床上进一步推广。Objective To explore the application of respiratory humidification therapy apparatus in sequential ventilation of respiratory failure caused by chronic obstructive pulmonary disease(COPD).Methods Forty patients with respiratory failure caused by COPD were randomly divided into observation group(20 cases)and control group(20 cases).The observation group was treated with respiratory humidification therapy apparatus,the control group was treated with traditional double loop oxygen therapy.The vital signs,blood gas analysis,the incidence of ventilatorassociated pneumonia(VAP),remechanical ventilation rate,hospital stays in ICU and hospital cost of the two groups before weaning and 24 hours after weaning were compared.Results Compared with the control group,the respiratory rate,arterial blood oxygen partial pressure and oxygenation index of the observation group were improved more significantly 24 hours after weaning(P<0.05).The duration of remechanical ventilation and hospitalization were shorter,and the total hospitalization cost was lower in the observation group.Conclusion Respiratory humidification therapy apparatus can improve the symptoms of patients with respiratory failure caused by COPD and reduce hospitalization cost,which is worthy of clinical application.
