摘　　要：目的:探究早发型重度子痫前期(ESPE)与晚发型重度子痫前期(LSPE)患者胎盘组织中staufen1蛋白(STAU1)、胎盘生长因子(PLGF)表达水平与妊娠结局关系。方法:选取2017年1月—2019年10月于本院分娩的重度子痫前期孕妇86例,其中早发型43例(ESPE组)、晚发型43例(LSPE组),另随机选取同期正常妊娠分娩孕妇43例为对照组,分别采用Western blotting蛋白印迹方法和免疫组织化学法检测各组胎盘组织中STAU1、PLGF表达并分析与妊娠结局关系。结果:ESPE组胎盘组织中STAU1(0.69±0.13)、PLGF(72.14±4.42)表达水平均低于LSPE组(1.25±0.24、120.31±6.45)和对照组(1.34±0.21、117.78±6.31)(P<0.05),LSPE组与对照组无差异(P>0.05)。胎盘重量(385.5±41.2g)、新生儿出生体重(1.21±0.23kg)、新生儿1min Apgar评分(8.01±0.76分)ESPE组均低于LSPE组和对照组,且LSPE组低于对照组(P<0.05)。Pearson相关性分析,ESPE与新生儿出生体重、新生儿1min Apgar评分、胎盘重量呈负相关(r=-0.441、-0.572、-0.615,均P=0.000)。结论:ESPE孕妇胎盘组织STAU1、PLGF显著低表达,而LSPE孕妇与正常孕妇差异不显著;STAU1、PLGF低表达可能与ESPE发病及进展有关。Objective:To study the expression level of staufen1 protein(STAU1)and placental growth factor(PLGF)in placenta of pregnant women with early onset severe preeclampsia(ESPE)or late onset severe preeclampsia(LSPE),and to study their relationship with pregnancy outcomes.Methods:86 pregnant women with preeclampsia from January,2017 to October,2019 were selected and divided into 43 women with ESPE in group A and 43 women with LSPE in group B,and other 43 normal pregnant women were randomly selected in group C during the same period.The expression levels of STAU1 and PLGF in placenta of these women were detected by Western blotting and immunohistochemistry respectively,and their relationship with pregnancy outcomes was analyzed.Results:The expression levels of STAU1(0.69±0.13)and PLGF(72.14±4.42)in placental tissues of women in group A were significant lower than those of women in group B(1.25±0.24 and 120.31±6.45)and those(1.34±0.21 and 117.78±6.31)of women in group C(P<0.05),but which of women had no significant difference between group B and group C(P>0.05).The placental weight(385.5±41.2g)of women,and birth weight(1.21±0.23kg),and 1 min Apgar score(8.01±0.76)of newborn in group A were all significant lower than those in group B and Group C,and those in group B were significant lower than those in group C(P<0.05).Pearson correlation analysis showed that ESPE was negatively correlated with neonatal birth weight,Apgar score,and placental weight(r=-0.441,-0.572,-0.615,all P=0.000).Conclusion:The expression levels of STAU1 and PLGF in placenta of pregnant women with ESPE have significantly decrease,but which have no significant difference between pregnant women with LSPE and normal pregnant women.The low expression of STAU1 and PLGF may be closely related to the pathogenesis and progress of women with ESPE.Women with ESPE have high risk of adverse pregnancy,so it should be paid more attention to in clinical practice.
