摘 要:目的:探讨改良经阴道实时三维子宫输卵管超声造影技术(TVS RT 3D-HyCoSy)联合宫腔水造影(SIS)诊断不孕症患者宫腔粘连的临床价值。方法:将2017年7月1日—2020年1月1日本院收治不孕症患者135例作为研究对象,对所有患者术前采用改良TVS RT 3D-HyCoSy联合SIS检查,并与腹腔镜检查(LPS)结果对比,观察改良TVS RT 3D-HyCoSy联合SIS诊断输卵管通畅性的灵敏度、特异度及准确度;然后再对所有患者宫腔镜检查(HS),观察改良TVS RT 3D-HyCoSy联合SIS与单用改良TVS RT 3D-HyCoSy诊断宫腔粘连的灵敏度、特异度及准确度,对比其诊断效能。结果:改良TVS RT 3D-HyCoSy联合SIS诊断输卵管通畅性的灵敏度(88.4%)、特异度(93.3%)及准确度(91.1%),诊断宫腔粘连的灵敏度(100.0%)、特异度(95.2%)及准确度(97.8%)高于单用改良TVS RT 3D-HyCoSy(78.3%、72.7%、75.6%)P<0.05)。结论:改良TVS RT 3D-HyCoSy联合SIS可更精准地评估不孕症患者宫腔粘连状态。Objective:To investigate the clinical value of modified transvaginal real-time three-dimensional hysterosalpingography by ultrasound(TVS RT 3D-HyCoSy)combined with hydro hysterosalpingography(HHS)for diagnosing intrauterine adhesions of infertility patients.Methods:From July 1,2017 to January 1,2020,135 infertility patients were selected as the research objects.All patients were examined by modified TVS RT 3D-HyCoSy combined with HHS for diagnosing their intrauterine adhesions before operation,and those results were compared to the results by laparoscopy(LPS).The sensitivity,the specificity,and the accuracy of the modified TVS RT 3D-HyCoSy combined with HHS for diagnosing fallopian tube patency of infertility patients were calculated.As for those infertility patients after hysteroscopy(HS),the sensitivity,the specificity,and the accuracy of the modified TVS RT 3D-HyCoSy combined with HHS or the modified TVS RT 3D-HyCoSy alone for diagnosing intrauterine adhesions were also calculated,and the diagnostic efficiency of them was compared.Results:The sensitivity,the specificity,and the accuracy of the modified TVS RT 3D-HyCoSy combined with HHS for diagnosing fallopian tube patency of infertility patients were 88.4%,93.3%,and 91.1%,respectively.The sensitivity(100.0%),the specificity(95.2%),and the accuracy(97.8%)of the modified TVS RT 3D-HyCoSy combined with HHS for diagnosing intrauterine adhesions were significant higher than those(78.3%,72.7%,and 75.6%)of the modified TVS RT 3D-HyCoSy alone P<0.05).Conclusion:The modified TVS RT 3D-HyCoSy combined with HHS can assess the state of intrauterine adhesions of infertility patients more accurately.
