通气技术对面罩机械通气治疗慢性阻塞性肺疾病合并呼吸衰竭疗效的影响研究

摘　　要：目的探讨慢性阻塞性肺病(COPD)合并呼吸衰竭应用面罩器械通气治疗时采用不同通气技术的疗效。方法对2017年1月~2020年5月期间在本院接受治疗的100例COPD合并呼吸衰竭患者进行研究,均采用面罩机械通气治疗。按抽签法抽取33例归为A组,采用S/T模式,使用橡胶面罩;33例归为B组,采用S/T模式,使用塑料面罩;34例归为C组,采用压力支持通气,观察三组治疗前后血气指标及生命体征相关指标变化情况。结果通气后,三组动脉氧分压(PaO2)、动脉二氧化碳分压(PaCO2)、血氧饱和度(SaO2)、pH值均有所改善(P<0.05),但组间比较差异无统计学意义(均P>0.05);通气后,三组收缩压(SBP)、心率(HR)、呼吸频率(RR)均有所改善(P<0.05),但组间比较差异无统计学意义(均P>0.05)。结论对COPD合并呼吸衰竭患者进行面罩机械通气治疗,能够显著改善患者的呼吸状况,同时应结合患者的情况选择合适的通气技术,以达到最佳的治疗效果。Objective To explore the effect of various ventilation techniques in the treatment of chronic obstructive pulmonary disease(COPD)patients with respiratory failure via mask mechanical ventilation.Methods A total of 100 COPD patients with respiratory failure who were treated in our hospital from January 2017 to May 2020 were selected as the research objects,and all patients were treated by mask mechanical ventilation.They are divided into Group A(n=33)and Group B(n=33)and Group C(n=34)by lot method.Group A was treated with S/T mode and rubber masks,Group B was treated with S/T mode and plastic masks and Group C was treated by pressure support ventilation.The changes of blood gases and vital signs among the three groups were observed before and after treatment.Results After ventilation,the arterial oxygen pressure(PaO2),arterial carbon dioxide pressure of blood(PaCO2),oxygen saturation of blood(SaO2)and pH value were all improved in the three groups(P<0.05),but there were no significant differences between the three groups(all P>0.05).After ventilation,systolic blood pressure(SBP),heart rate(HR),and respiratory rate(RR)were all improved in the three groups(P<0.05),but there were no significant differences between the three groups(all P>0.05).Conclusion Mask mechanical ventilation can significantly improve the respiratory condition of COPD patients with respiratory failure.Meanwhile,appropriate ventilation techniques should be selected based on the patient's condition to achieve the best therapeutic effect.
