初诊糖尿病及糖尿病前期早相胰岛分泌功能情况的调查

摘　　要：目的探讨初诊糖尿病及糖尿病前期患者早相胰岛分泌功能的情况及其影响因素。方法2019年1~6月从我院体检中心随机抽取龙岩市社区500例既往无糖尿病史的体检居民进行调查,测定其空腹及OGTT试验0.5 h、2 h血糖、胰岛素、血脂,记录BMI、腰臀比(WHR)、年龄、平均收缩压(SBP)、平均舒张压(DBP),计算早相胰岛分泌功能指数(△I30/△G30)、胰岛素抵抗指数(HOMA-IR)。结果早相胰岛分泌功能,初诊DM组<IGT+IFG组<IGT组<IFG组<NGT组;胰岛素抵抗,初诊DM组>IGT+IFG组>IFG组>IGT组>NGT组。多元逐步回归分析显示,WHR、SBP、TG、HOMA-IR与△I30/△G30呈显著负相关,HLD-C与△I30/△G30呈显著正相关(P<0.05);BMI、WHR、SBP、LDL-C与HOMA-IR呈显著正相关,而HLD-C与HOMA-IR呈显著负相关。结论IFG、IGT、IGT+IFG、初诊DM的早相胰岛分泌功能呈进行性受损,其与胰岛素抵抗水平有关,控制BMI、WHR、血脂、收缩压达标具有重要意义。Objective To explore the early-phase pancreatic islet secretory function and its influencing factors in newly diagnosed diabetes and prediabetes patients.Methods A total of 500 residents in the community of Longyan who had no history of diabetes from January 2019 to June 2019 were randomly selected from the physical examination center of our hospital for investigation.Their fasting and OGTT test 0.5 h,2 h blood glucose,insulin,and blood lipids were measured.The BMI,Waist-hip ratio(WHR),age,mean systolic blood pressure(SBP),and mean diastolic blood pressure(DBP)were recorded.The early phase pancreatic islet secretory function index(△I30/△G30),insulin resistance index(HOMA-IR)were calculated.Results Early-phase islet secretion function,newly diagnosed DM group<IGT+IFG group<IGT group<IFG group<NGT group;insulin resistance,newly diagnosed DM group>IGT+IFG group>IFG group>IGT group>NGT group.Multiple stepwise regression analysis showed that WHR,SBP,TG,HOMA-IR were significantly negatively correlated with△I30/△G30,and HLD-C was significantly positively correlated with△I30/△G30(P<0.05);BMI,WHR,SBP,LDL-C has a significant positive correlation with HOMA-IR,while HLD-C has a significant negative correlation with HOMA-IR.Conclusion IFG,IGT,IGT+IFG,and newly diagnosed DM have progressively impaired pancreatic islet secretion in the early phase,which is related to the level of insulin resistance.It is of great significance to control BMI,WHR,blood lipids,and systolic blood pressure.
