摘  要:目的探讨泪腺MR DWI在甲状腺相关性眼病(TAO)诊断和分期中的价值.方法回顾性分析接受DWI检查的61例TAO患者(TAO组)及30例健康志愿者(对照组).在ADC图像上采用ROI法测量泪腺的ADC值,同时测量脑干区的ADC值,以获得泪腺校正ADC值(泪腺ADC/脑干ADC).采用t检验比较TAO组与对照组间及活动期组与非活动期组间泪腺ADC及校正ADC的差异.TAO组泪腺ADC及校正ADC与临床活动度评分(CAS)的相关性采用Spearman's相关分析.结果TAO组泪腺ADC(P=0.011)及校正ADC(P=0.005)均大于对照组.同时,活动期组泪腺ADC(P=0.001)及校正ADC(P=0.004)均大于非活动期组.相关性分析结果显示,CAS与泪腺ADC(r=0.543,P<0.001)及校正ADC(r=0.510,P=0.001)均呈正相关.结论泪腺DWI可反映TAO患者泪腺组织的扩散特性,泪腺ADC值可以辅助TAO的临床诊断和分期,为临床治疗计划的制订提供依据.Objective To assess the value of lacrimal glands in diagnosis and staging of thyroid associated ophthalmopathy(TAO)with MR DWL.Methods A total of 61 patients with TAO(TAO group)and 30 healthy volunteers(control group)who underwent DWI were analyzed retrospectively.ADC values of lacrimal glands were measured with ROI method,ADC values were also measured in the brainstem region and corrected ADC values(lacrimal gland ADC/brainstem ADC)were calculated.Independent samples t test was used for comparing the ADC and corrected ADC between TAO group and control group,active TAO and inactive TAO.Spearman's correlation test was used to analyze the correlation of ADC values and corrected ADC values with the clinical activity score(CAS)in TAO group.Results The ADC values(P=0.011)and corrected ADC values(P=0.005)of lacrimal glands in TAO group were significantly higher than those in control group.Meanwhile,lacrimal glands in active TAO showed significantly higher ADC values(P=0.001)and corrected ADC values(P=0.004)than those in inactive TAO.Both the ADC values(r=0.543,P<0.001)and corrcted ADC values(r=0.510,P=0.001)of lacrimal glands in TAO group had positive correlations with CAS.Conclusion DWI can reflect the diffusion characteristic of lacrimal glands.Its derived ADC values can assist in the diagnosis and staging of TAO and provide a basis for the clinical treatment plans.
