摘　　要：目的通过对类风湿关节炎(RA)/肺间质病变(ILD)患者进行高分辨率CT(HRCT)联合血清学指标检测,考察阿达木单抗对RA/ILD患者的临床疗效.方法选取在风湿科诊断为RA/ILD患者184例为研究对象,随机分为:对照组(来氟米特,94例),试验组(阿达木单抗,90例).对比分析2组前后以及组间临床疗效差异,主要从以下2方面评价:(1)肺功能指标,第1秒用力呼气容积(FEV1),用力肺活量(FVC),总肺量(TLC),HRCT评分.(2)血清学指标,类风湿因子(RF),C反应蛋白(CRP),低氧诱导因子1α(HIF-1α),白介素-22(IL-22).结果试验组临床疗效明显高于对照组.肺功能指标FEV1、FVC、TLC、HRCT评分显著高于对照组.血清学指标RF、CRP、HIF-1α、IL-22显著低于对照组.结论HRCT联合血清学指标诊断显示阿达木单抗对RA/ILD患者临床疗效明显高于传统药物.Objective To investigate the clinical efficacy of adamumab in patients with rheumatoid(RA)/interstitial lung disease(ILD)by high resolution CT(HRCT)combined with serological indicators in RA/ILD patients.Methods 184 cases diagnosed as RA/ILD were selected as the study subjects and randomly divided into control group(leflunomide,94 cases)and experimental group(adamumab,90 cases).The difference of clinical efficacy between the two groups before and after treatment and between the two groups were compared and analyzed.The following aspects were mainly evaluated:(1)Pulmonary function indicators:forced expiratory volume in one second(FEV1),forced vital capacity(FVC),total lung volume(TLC),HRCT score.(2)Serological indicators:rheumatoid factor(RF),C-reactive protein(CRP),hypoxia inducible factor 1 alpha(HIF-1α),interleukin-22(IL-22).Results The clinical eficacy of adamumab in the experimental group was significantly higher than that in the control group.Lung function indexes such as FEV1,FVC,TLC and HRCT scores were significantly higher than those of the control group.Serological indicators such as RF,CRP,HIF-1αand IL-22 were significantly lower than those of in the control group.Conclusion The diagnosis of HRCT combined with serological indicators shows that the clinical efficacy of adamumab in patients with RA/ILD is significantly higher than that of traditional drugs.
