摘　　要：目的探讨部分性脾动脉栓塞术(PSE)对门静脉高压性脾功能亢进患者免疫功能的影响.方法选取PSE治疗门静脉高压性脾功能亢进患者,比较术前、术后患者外周血细胞、体液免疫及细胞免疫资料.结果白细胞计数(WBC)、血小板计数(PLT)术后7 d、30 d、90 d、180 d与术前比较有统计学差异(P<0.05);红细胞计数(RBC)术后180 d与术前比较有统计学差异(P<0.05);CD3术后90 d、180 d与术前比较有统计学差异(P<0.05);CD4、CD8及CD4/CD8术后30 d、90 d、180 d与术前比较有统计学差异(P<0.05);IgA术后30 d、90 d与术前比较有统计学差异(P<0.05);IgM术后180 d与术前比较有统计学差异(P<0.05);IgG术后30 d、90 d、180 d与术前比较有统计学差异(P<0.05).结论PSE可有效改善患者脾功能亢进症状,同时保留部分脾脏功能,提升外周血细胞数,改善机体免疫功能.Objective To investigate the ffct of partial splenie embolization(PSE)on immune function in patients with portal hypertension with hypersplenism.Methods PSE was performed in patients with hypersplenism caused by portal hypertension.Data of peripheral blood cells,humoral immunity and ellular immunity were compared before and after surgery.Results There were statistically significant differences(P<0.05)in white blood cell count(WBC),blood platelet count(PLT)7 days,30 days,90 days and 180 days after operation compared with that before operation;there were statistically significant differences(P<0.05)in red blood cell count(RBC)180 days after operation compared with that before operation;there were statistically sigificant differences(P<0.05)in CD390 days,180 days after operation compared with that before operation;there were staistically significant dfferences(P<0.05)in CD4,CD8 and CD4/CD830 days,90 days and 180 days after operation compared with that before operation.There was statistical difference(P<0.05)between 30 days after operation and 90 days after operation and before operation in IgA;there was statistical difference(P<0.05)between 180 days after operation and before operation in IgM;there was statistical difference(P<0.05)between 30 days after operation,90 days after operation and 180 days after operation and before operation in 1gG.Conclusion PSE can effectively improve the symptoms of patients with hypersplenism,retain part of spleen function,improve the number of peripheral blood cells,and improve the immune function of the body.
