温针灸联合洛索洛芬钠对膝关节骨性关节炎急性加重期患者骨代谢及IL-6、IL-1、SOD的影响

摘　　要：目的:探讨温针灸结合洛索洛芬钠对膝关节骨性关节炎急性加重期患者骨代谢指标及血清白细胞介素6(interleukin-6,IL-6)、白细胞介素1(interleukin-1,IL-1)、超氧化物歧化酶(superoxide dismutase,SOD)的影响。方法:将膝关节骨性关节炎患者112例采用随机数字表法分为观察组与对照组各5 6例,对照组给予洛索洛芬钠治疗,观察组在对照组基础上结合温针灸治疗。比较两组患者临床疗效,骨代谢指标及血清IL-6、IL-1、SOD水平。结果:观察组总有效率为96.4%(54/56),高于对照组的76.8%(43/56)(P<0.05);治疗后两组患者Lysholm膝关节评分均较治疗前增高(P<0.05),lequesne MG评分均较治疗前降低(P<0.05),观察组改善优于对照组(P<0.05);治疗后两组患者BGP、OPG、IGF-1,FGF-2均较治疗前升高(P<0.05),观察组高于对照组(P<0.05);两组患者CT值治疗前后及治疗后比较差异无统计学意义(P>0.05);治疗后两组患者IL-6、IL-1水平均较治疗前下降(P<0.05),SOD水平较治疗前升高(P<0.05),且观察组改善优于对照组(P<0.05)。结论:温针灸联合洛索洛芬钠可提高膝关节骨性关节炎急性加重期患者临床疗效,提高患者骨代谢指标,抑制炎症因子,改善关节功能。Objective: To observe the influences of warming acupuncture combined with loxoprofen sodium on the bone metabolism indexes and serum IL-6, IL-1 and SOD in acute exacerbation of knee osteoarthritis(KOA).Methods: All 112 KOA patients were divided into the observation group and the control group according to random number table method, 56 cases each group, the control group were treated by loxoprofen sodium, and the observation group by warming acupuncture on the foundation of the therapy the control group accepted. To compare clinical effects, bone metabolism indexes and the levels of serum IL-6, IL-1 and SOD between both groups. Results: Total effective rate of the observation group was 96.4%(54/56), higher than 76.8%(43/56) of the control group(P <0.05);Lysholm knee joint scores of both groups after treating increased compared with these before treating(P <0.05),lequesneMG scores reduced than before treating(P<0.05), the improvements of the observation group were superior to these of the control group(P<0.05);after treating, BGP, OPG, IGF-1 and FGF-2 of both groups lifted compared with before treating(P<0.05), the observation group was higher than the control group(P<0.05);the difference had no statistical meaning in CT values before and after treating, between both groups after treating(P>0.05);the levels of IL-6 and IL-1 of both groups after treating were lower than these before treating(P<0.05), the levels of SOD rose than before treating(P<0.05), and the improvements of the observation group were superior to these of the control group(P <0.05). Conclusion: Warming acupuncture combined with loxoprofen sodium could lift clinical effects,raise the bone metabolism indexes, inhibit inflammatory factors and improve joint function in acute exacerbation of KOA.
