序贯给药与持续股神经阻滞对全膝关节置换术患者镇痛效果对比

摘　　要：目的:探讨序贯给药同持续股神经阻滞用于全膝关节置换术中的镇痛效果差异。方法:2018年1月-2019年5月收治行全膝关节置换术患者86例,随机分为两组,各43例。对照组采用持续股神经阻滞麻醉,观察组采用序贯给药麻醉,比较两组患者术后1 d、2 d、3 d疼痛评分、膝关节功能情况及术后不良反应情况。结果:观察组术后1 d、2 d、3 d VAS评分低于对照组,1周、2周、3周HSS评分高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:临床全膝关节置换术患者术后镇痛落实序贯给药较常规持续股神经阻滞镇痛有助于进一步减轻疼痛,促进早期康复训练,且不良反应发生率更低。Objective:To analyze the difference between the analgesic effect of sequential administration and continuous bone nerve block in total knee arthroplasty.Methods:From January 2018 to May 2019,86 patients underwent total knee arthroplasty were selected,they were randomly divided into the two groups with 43 cases in each group.The control group used continuous femoral nerve block anesthesia.The observation group used sequential administration anesthesia.The pain score,knee joint function and postoperative adverse reactions were compared between the two groups at 1 day,2 days and 3 days after operation.Results:The VAS score of the observation group was lower than that of the control group at 1 d,2 d and 3 d after operation,and the HSS score at 1 week,2 week and 3 week was higher than that of the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:Sequential administration of postoperative analgesia in patients undergoing clinical total knee arthroplasty is more effective than conventional continuous femoral nerve block analgesia,which can further reduce pain,promote early rehabilitation training,and has a lower incidence of adverse reactions.
