观察温针灸对复发性流产患者黄体中期子宫动脉血流及子宫内膜容受性的影响研究

摘　　要：目的:观察温针灸对复发性流产患者子宫动脉血流及子宫内膜容受性的影响。方法:选取2019年4月-2020年3月在生殖内分泌门诊就诊的育龄期妇女38例,根据病史分为A组(对照组)及B组(自然流产≥2次)。应用经阴道彩色多普勒超声分别检测两组患者黄体中期子宫动脉厚度和血流指数(S/D值、PI值、RI值)。B组患者于月经第2天开始行温针灸治疗1个疗程,观察治疗前后黄体中期的子宫内膜厚度及子宫动脉血流指数。结果:B组自然周期黄体中期子宫内膜厚度显著小于A组,差异有统计学意义(P<0.05);B组治疗后子宫内膜厚度增加,差异有统计学意义(P<0.05)。B组治疗前子宫动脉血流指数均显著高于A组,差异有统计学意义(P<0.05);B组治疗后子宫动脉血流阻力得到明显改善,差异有统计学意义(P<0.001)。结论:温针灸可有效改善子宫内膜厚度及子宫动脉血流灌注,从而在一定程度上改善自然流产患者的子宫内膜容受性。Objective:To observe the effects of warm acupuncture on uterine arterial blood flow and endometrial receptivity in patients with recurrent abortion.Methods:From April 2019 to March 2020,38 women of childbearing age who visited the reproductive endocrine clinic were selected and divided into group A(control group)and Group B(spontaneous abortion≥2 times)according to the medical history.Transvaginal color Doppler ultrasound was used to detect the middle luteal uterine artery thickness and blood flow index(S/D value,PI value and RI value)in the two groups.Patients in group B received warm acupuncture treatment for one course on the second day of menstruation,and the endometrial thickness and uterine artery blood flow index were observed in the middle luteal period before and after treatment.Results:The endometrial thickness in the middle luteal phase of natural cycle in group B was significantly lower than the group A,with the difference was statistically significant(P<0.05).Endometrial thickness increased after treatment in group B,with the difference was statistically significant(P<0.05).Before treatment,uterine artery blood flow index in group B was significantly higher than the group A,with the difference was statistically significant(P<0.05).Uterine artery blood flow resistance was significantly improved after treatment in group B,with the difference was statistically significant(P<0.05).Conclusion:Warm acupuncture and moxibustion can effectively improve endometrial thickness and uterine artery blood perfusion,so as to improve the endometrial receptivity of spontaneous abortion patients to a certain extent.
