摘  要:目的探讨无痛分娩对COOK球囊引产效果和母婴结局的影响。方法选取2017年8月~2019年8月于广东省吴川市妇幼保健计划生育服务中心行COOK球囊引产的190例足月初产妇作为研究对象,采用抽签分组法将其分为对照组与观察组,每组各95例。对照组产妇予以常规分娩方式,观察组产妇予以无痛分娩。比较两组产妇的宫颈成熟度、疼痛程度、妊娠结局、产程时间及不良母婴结局。结果两组产妇干预前后的宫颈成熟度评分(Bishop评分)比较,差异无统计学意义(P>0.05)。观察组产妇干预后的视觉模拟量表(VAS)评分低于对照组,差异有统计学意义(P<0.05)。观察组产妇的自然分娩率高于对照组,剖宫产率低于对照组,差异有统计学意义(P<0.05)。观察组自然分娩产妇的第一产程用时短于对照组,差异有统计学意义(P<0.05)。观察组的不良母婴结局总发生率低于对照组,差异有统计学意义(P<0.05)。结论无痛分娩应用于COOK球囊引产的产妇中可减轻疼痛、提高自然分娩率、缩短产程时间及改善母婴结局。Objective To investigate the effect of painless labor on the induction of COOK balloon and maternal and neonatal outcomes.Methods A total of 190 full-term primiparous women who underwent COOK balloon induction in Maternal and Child Health Family Planning Service Center in Wuchuan City,Guangdong Province from August 2017 to August 2019 were selected as the research objects and divided into the control group and the observation group according to the lottery grouping method,with 95 cases in each group.The control group was given routine delivery,and the observation group was given painless delivery.Cervical maturity,pain,pregnancy outcome,labor time,adverse maternal and neonatal outcomes were compared between the two groups.Results There was no significant difference in cervical maturity(Bishop)score between the two groups before and after intervention(P>0.05).The visual analogue scale(VAS)score of the observation group after intervention was lower than that of the control group,and the difference was statistically significant(P<0.05).The natural delivery rate in the observation group was higher than that in the control group,the cesarean delivery rate was lower than that in the control group,and the differences were statistically significant(P<0.05).The time of the first labor of the spontaneous labor women in observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse maternal and neonatal outcomes in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Painless delivery in COOK balloon induced labor can reduce pain,increase the rate of natural delivery,shorten the labor time and improve maternal and child outcomes.
