摘  要:目的探讨新冠肺炎疫情下骨科与急诊外科联合管理治疗老年椎体压缩性骨折的手术效果及安全性。方法回顾性分析2020年3~4月采用骨科与急诊外科联合管理的30例老年椎体压缩性骨折患者临床资料作为联合组,纳入既往非疫情期间30例老年椎体压缩性骨折患者作为对照组。记录两组术前等待时间、住院时间、手术时长、骨水泥注入量、术后视觉模拟量表(VAS)评分、术后Cobb角,并观察术后并发症发生情况。结果无医护人员感染新冠肺炎,未发生院内感染;联合组术前等待时间短于对照组,差异有统计学意义(P<0.05);两组住院时间、手术时长、骨水泥注入量、术后VAS评分、术后Cobb角、术后并发症总发生率比较,差异无统计学意义(P>0.05)。结论新冠肺炎疫情下,实施骨科与急诊外科联合管理治疗老年椎体压缩性骨折,多学科协作,可以保证防疫措施落实到位,同时可缩短术前等待时间,提高诊疗效率。Objective To investigate the surgical effect and safety of combined management of orthopedics and emergency surgery in the treatment of elderly vertebral compression fractures under the COVID-19 pandemic.Methods A retrospective analysis of the clinical data of 30 elderly patients with vertebral compression fractures under the joint management of orthopedics and emergency surgery from March to April 2020 was conducted and selected as a joint group.Another batch of 30 elderly patients with vertebral compression during the non-epidemic period was classified as a control group.The preoperative waiting time,hospital stay,operation duration,bone cement injection volume,postoperative visual analogue scale(VAS)score,postoperative Cobb angle were recorded in the two groups,and the occurrence of postoperative complications was observed.Results No medical staff was infected with COVID-19,and no hospital infection occurred.The preoperative waiting time of the joint group was shorter than that of the control group,and the difference was statistically significant(P<0.05).The hospital stay,operative duration,bone cement injection volume,postoperative VAS score,postoperative Cobb angle,and total postoperative complications rate were not significantly different between the groups(P>0.05).Conclusion Under the COVID-19 pandemic,the joint management of orthopedics and emergency surgery for the treatment of elderly vertebral compression fractures in multidisciplinary collaborations can reduce the waiting time before surgery and increase the efficiency of diagnosis and treatment at premise of fully implementing anti-epidemic measures.
