气管镜下滴药治疗支气管结核的效果分析

摘　　要：目的:分析气管镜下滴药治疗支气管结核(EBTB)的效果。方法:选取2016年10月-2019年6月在笔者所在医院治疗的49例EBTB患者。根据治疗方法分为两组,在常规全身抗结核药基础上,A组行气管镜下滴药治疗,B组行常规雾化治疗(两组药物均为异烟肼注射液和阿米卡星混合液)。对比两组临床疗效。结果:A组总有效率(96.0%)高于B组(75.0%),症状消退时间、基本治愈时间均短于B组,并发症发生率(12.0%)低于B组(37.5%),随访3、9个月痰菌阴转率(52.0%,92.0%)均高于B组(20.8%,66.7%),治疗后T细胞亚群水平均优于B组,差异均有统计学意义(P<0.05)。结论:为EBTB患者行气管镜下滴药治疗可改善疗效与机体免疫功能,促进痰菌阴转,缩短治疗周期,且安全性较好。Objective:To analyze the effect of bronchoscopic drug dropping on bronchial tuberculosis(EBTB).Method:A total of 49 EBTB patients treated in the author’s hospital from October 2016 to June 2019 were selected.According to the treatment methods,they were divided into two groups.On the basis of conventional systemic anti-tuberculosis drugs,group A was treated with bronchoscopic drug dropping,and group B was treated with conventional atomization treatment(both groups were treated with Isoniazid Injection and Amikacin Mixture).The clinical efficacy of the two groups was compared.Result:The total effective rate in group A(96.0%)was higher than that in group B(75.0%),the time for symptom resolution and complete cure were shorter than those in group B,the complication rate in group A was lower than that in group B,sputum negative conversion rates(52.0%,92.0%)were higher than those in group B(20.8%,66.7%)during 3 and 9 months of follow-up,and the indicators of T cell subsets in group A were better than those in group B,the differences were statistically significant(P<0.05).Conclusion:Bronchoscopic drug dropping therapy for EBTB patients can improve the curative effect and immune function of the body,promote the negative conversion of phlegm bacteria,shorten the treatment cycle,and has good safety.
