无创呼吸机联合布地奈德福莫特罗治疗SAHS合并COPD急性加重期的效果

摘　　要：目的:探讨无创呼吸机联合布地奈德福莫特罗治疗睡眠呼吸暂停低通气综合征(SAHS)合并慢性阻塞性肺疾病(COPD)急性加重期重叠综合征的效果。方法:选择2016年7月10日-2019年7月30日本院收治的72例SAHS合并COPD急性加重期重叠综合征患者开展此次研究,随机分成对照组(36例)与研究组(36例)。对照组单独采取布地奈德福莫特罗粉吸入剂治疗,研究组采取无创呼吸机联合布地奈德福莫特罗粉吸入剂治疗,对比两组治疗前后的肺功能指标、血气分析指标。结果:研究组治疗总有效率为94.44%,显著高于对照组的75.00%(P<0.05);治疗后,研究组呼气峰值流速(PEF)、第一秒用力呼气容积(FEV1)、FEV1/FVC(用力肺活量)、动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)水平均显著优于对照组(P<0.05),两组氢离子浓度指数(pH)水平比较,差异无统计学意义(P>0.05)。结论:采取无创通气结合布地奈德福莫特罗治疗SAHS合并COPD急性加重期重叠综合征患者,疗效显著,可有效改善患者的肺功能,缓解呼吸困难症状,值得推广应用。Objective:To investigate the effect of noninvasive ventilator combined with Budesonide Formoterol in the treatment of sleep apnea hypopnea syndrome(SAHS)and chronic obstructive pulmonary disease(COPD)acute exacerbation overlap syndrome.Method:From July 10,2016 to July 30,2019,72 patients with SAHS complicated with COPD acute exacerbation overlap syndrome were selected and randomly divided into the control group(36 cases)and the study group(36 cases).The control group was treated with Budesonide Formoterol inhaler alone,while the study group was treated with noninvasive ventilator combined with Budesonide Formoterol inhaler.The pulmonary function indexes and blood gas analysis indexes before and after treatment were compared between the two groups.Result:The total effective rate of the study group was 94.44%,significantly higher than 75.00%of the control group(P<0.05).After treatment,the peak expiratory flow rate(PEF),forced expiratory volume(FEV1),FEV1/FVC(forced vital capacity),arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)of the study group were better than those before treatment(P<0.05).Comparison of hydrogen ion concentration index(pH)level between the two groups showed no statistically significant difference(P>0.05).Conclusion:Noninvasive ventilation combined with Budesonide Formoterol in the treatment of SAHS complicated with COPD acute exacerbation overlap syndrome has significant curative effect,which can effectively improve the lung function of patients,and alleviate the symptoms of dyspnea,which is worthy of popularizing and applying.
