态靶辨证在郁火上炎型慢性咽炎中的运用——升阳散火汤加冬凌草、桔梗、生甘草

摘　　要：目的慢性咽炎相对应于传统中医"慢喉痹",临床非常常见,有发病率高、病程缠绵难愈等特点。现代人工作生活压力大,长期忧思焦虑,或贪凉饮冷,致脾胃气虚,阳气抑遏,形成郁火,上壅于咽喉,成为慢喉痹常见病因之一。运用仝小林院士"态靶结合"辨证组方思想治疗郁火上炎型喉痹,临床上取得很好的疗效。仝院士以郁火上炎为态,咽喉不利为靶,运用李东垣"补其中,升其阳,甘寒以泻其阴火"的原则,升阳散火汤以大量风药发散、宣通气机,疏郁、升阳,用之消散郁火,合用补脾益气之药以助中焦脾胃之运化以治本,全方散郁火、补中气、恢复中焦气机运转以调态,加用冬凌草、桔梗、甘草等清热利咽药物打靶,标本同治,郁火得散,咽喉得清利,运用于临床,疗效显著。Chronic pharyngitis corresponds to"Manhoubi"in traditional Chinese medicine.Itis very common in clinical practice,and has the characteristics of high incidence and is a refractory protracted disease.Urban peopleare usuallyunder great pressure in work and life,long-term anxiety or cold drinks,which can cause Qi deficiency of spleen and stomach,Yang Qi suppressionand stagnation in the throat,becoming one of the common causes of chronic pharyngitis.Professor Tong Xiaolin′s"combination of state and target"syndrome differentiation and treatmenthas played a good role in the differentiation and treatment of stagnation of fire and inflammation type of chronic pharyngitis.Professor TONG takes the depression of fire and inflammation as the stateandsore throat as the target.According toLI Dongyuan′s principle of"replenishingthe middle,raisingYang,and reducingYin and fire",Shengyang Sanhuo Decoction uses large amount of wind drugs to disperseand relieve depression and raise Yangso as todissipate stagnant fire,combiningdrugs forregulatingspleen and stomach,the whole prescription can disperse the stagnant fire,invigorate the middle Qiand restore the operation of the middle Jiao to adjust the state.Adding Donglingcao(Rabdosia rubescens),Jiegeng(Platycodon grandiflorus)and Gancao(Glycyrrhiza uralensis)can treat the throat as target.This method treats the symptoms and state at the same time and has a good therapeutic effect.
