五加生化胶囊与新生化冲剂治疗女性药流后阴道出血的临床随机对照研究

摘　　要：目的探讨五加生化胶囊与新生化冲剂治疗女性药流后阴道出血的临床随机对照研究。方法将2015年2月2018年6月该院女性药流后阴道出血病例86例,随机分两组,各43例,对照组采用乙烯雌酚联合安宫黄体酮治疗联合五加生化胶囊(2.4 g/次,2次/d);观察组采用采用乙烯雌酚联合安宫黄体酮治疗联合新生化冲剂(12 g/次,3次/d)治疗,持续治疗2周为1个疗程。比较两组患者的临床疗效,记录阴道出血量、阴道出血时间及腹痛程度评分,治疗前后检测血肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、雌二醇(Estradiol,E2)、孕酮(progesterone,P)等,记录并发症状况。结果两组患者年龄、妊娠天数、月经周期及月经天数比较,差异无统计学意义(P>0.05)。对照组有效率81.40%(35/43)低于观察组有效率95.35%(41/43),差异有统计学意义(P<0.05)。对照组在阴道出血量、阴道出血时间及腹痛程度评分上,均高于观察组,差异有统计学意义(P<0.05)。与治疗前比,两组治疗后血E2升高,血P、TNF-α降低;与对照组比,观察组治疗后血E2较高,血P、TNF-α较低,差异有统计学意义(P<0.05)。两组患者并发症11.63%(5/43)与13.95%(6/43)比较,无统计学意义(P>0.05)。结论与五加生化胶囊比较,新生化冲剂治疗女性药流后阴道出血的疗效较好,缩短出血时间,降低血TNF-α、孕酮的分泌,提高E2,且安全性高。Objective To explore the effect of Wujia Shenghua Capsule and Xinshenghua Granule in the treatment of vaginal hemorrhage after drug abortion in women.Methods Eighty-six cases of vaginal bleeding after drug abortion in our hospital from February 2015 to June 2018 were randomly divided into two groups,43 cases in each group.The control group received ethylestradiol,progesterone combined with Wujia Shenghua Capsule(2.4 g/time,twice a day).The observation group received ethylestradiol,progesterone combined with Xinhua Granule(12 g/time,three times a day),and two groups were treated for two weeks as a course of treatment.The clinical effects of the two groups were compared,levels of vaginal bleeding volume,time of vaginal bleeding and abdominal pain score were recorded.Blood tumor necrosis factor-alpha(TNF-alpha),estradiol(E2)and progesterone(P)were measured before and after treatment,and complications were recorded.Results There was no significant difference in age,gestation days,menstrual cycle or menstrual days between the two groups(P>0.05).The effective rate of the control group was 81.40%(35/43),which was lower than that of the observation group(95.35%,41/43)(P<0.05).The amount of vaginal bleeding,the time of vaginal bleeding and the degree of abdominal pain in the control group were higher than those in the observation group,and the difference was statistically significant(P<0.05).Compared with those before treatment,blood E2 increased and blood P and TNF-alpha decreased in both groups after treatment.Compared with those of the control group,blood E2 was higher in the observation group after treatment,while blood P and TNF-alpha were lower in the observation group(P<0.05).Complications in the two groups were 11.63%(5/43)and 13.95%(6/43),respectively with no statistical significance(P>0.05).Conclusion Compared with Wujia Shenghua Capsule,Xinshenghua Granule is effective in the treatment of vaginal bleeding after drug abortion.They can shorten the bleeding time,reduce the secretion of TNF-alpha and progesteronean
