冠心病诊断中CTA的应用价值研究

摘　　要：目的探究冠心病诊断中冠状动脉(冠脉)CT血管造影(CTA)的应用价值。方法 86例冠心病患者,分别施予冠脉CTA诊断以及冠脉造影诊断。比较两种方法诊断血管狭窄程度结果、病变分支结果、阳性检出率。结果冠脉CTA诊断血管狭窄程度<30%、狭窄程度30%~70%、狭窄程度>70%占比分别为13.2%、62.3%、24.5%,与冠脉造影的14.1%、61.7%、24.2%比较,差异均无统计学意义(P>0.05)。经冠脉CTA诊断出阳性189条,其中左冠主干38条、左前降支62条、左旋回支50条、右冠脉39条;经冠脉造影诊断出阳性192条,其中左冠主干38条、左前降支63条、左旋回支51条、右冠脉40条。两种方法诊断病变分支阳性结果比较,差异无统计学意义(P>0.05)。冠脉CTA阳性检出率95.3%与冠脉造影的96.5%比较,差异无统计学意义(P>0.05)。结论冠脉CTA检查在冠心病的临床诊断中有着较高的应用价值,能够有效提升临床对冠心病的诊断效率。Objective To investigate the practical value of coronary CT angiography(CTA)in the diagnosis of coronary heart disease.Methods 86 patients with coronary heart disease were diagnosed with coronary CTA and coronary angiography.The diagnosis results of vascular stenosis degree and lesion branch diagnosis,and positive detection rate of the two methods were compared.Results The proportion of stenosis degree<30%,stenosis degree 30%-70%and stenosis degree>70%diagnosed by coronary CTA were 13.2%,62.3%and 24.5%respectively,which had no statistically significant difference compared with 14.1%,61.7%and 24.2%of coronary angiography(P>0.05).A total of 189 positives were diagnosed by coronary CTA,including 38 left coronary trunks,62 left anterior descending branches,50 left circumflex branches,and 39 right coronary arteries;a total of 192 positives were diagnosed by coronary angiography,including 38 left coronary artery,63 left anterior descending branch,51 left circumflex branch,and 40 right coronary artery.There was no statistically significant difference in the positive results of the two methods for diagnosing the diseased branch(P>0.05).The positive detection rate of coronary CTA was 95.3%,which had no statistically significant difference compared with 96.5%of coronary angiography(P>0.05).Conclusion Coronary CTA has a high application value in the clinical diagnosis of coronary heart disease,and it can effectively improve the clinical diagnosis efficiency of coronary heart disease.
