复方黄芪汤加减联合骨化三醇治疗糖尿病肾病的效果研究

摘　　要：目的探讨复方黄芪汤加减联合骨化三醇治疗糖尿病肾病的效果。方法 90例糖尿病肾病患者,随机分为实验组与对照组,各45例。对照组应用骨化三醇软胶囊治疗,实验组在对照组基础上加用复方黄芪汤加减治疗。比较两组患者临床疗效、治疗前后血糖水平[空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)]及肾功能水平[肌酐(Scr)、血清尿素氮(BUN)、尿微量白蛋白(ALB)]。结果实验组总有效率为93.33%,明显高于对照组的75.56%,差异具有统计学意义(P<0.05)。治疗后,实验组患者FBG(6.09±1.45)mmol/L、2 h PG(6.92±1.36)mmol/L、HbA1c(6.01±1.24)%均显著低于对照组的(7.42±1.73)mmol/L、(8.96±1.74)mmol/L、(7.42±1.76)%,差异均具有统计学意义(P<0.05)。治疗后,实验组患者Scr(83.46±10.25)μmol/L、BUN(6.45±0.94)mmol/L、ALB(0.35±0.08)mg/ml均显著低于对照组的(88.71±12.45)μmol/L、(7.52±1.07)mmol/L、(0.87±0.16)mg/ml,差异均具有统计学意义(P<0.05)。结论针对糖尿病肾病患者应用复方黄芪汤加减联合骨化三醇治疗效果显著,能够降低血糖水平,改善肾功能,值得推广应用。Objective To discuss the effect of modified compound Huangqi decoction and calcitriol on diabetic nephropathy.Methods A total of 90 patients with diabetic nephropathy were randomly divided into experimental group and control group,with 45 cases in each group.The control group was treated with calcitriol soft capsule,and the experimental group was treated with addition and subtraction of compound Huangqi decoction on the basis of the control group.The clinical efficacy,blood glucose levels[fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PG),glycosylated hemoglobin(HbA1 c)]and renal function levels[creatinine(Scr),blood urea nitrogen(BUN),urine albumin(ALB)]before and after treatment were compared between the two groups.Results The total effective rate of the experimental group was 93.33%,which was obviously higher than 75.56%of the control group,and the difference was statistically significant(P<0.05).After treatment,FBG(6.09±1.45)mmol/L,2 h PG(6.92±1.36)mmol/L and HbA1 c(6.01±1.24)%of the experimental group were significantly lower than(7.42±1.73)mmol/L,(8.96±1.74)mmol/L and(7.42±1.76)%of the control group,and the difference was statistically significant(P<0.05).After treatment,Scr(83.46±10.25)μmol/L,BUN(6.45±0.94)mmol/L and ALB(0.35±0.08)mg/ml of the experimental group were significantly lower than(88.71±12.45)μmol/L,(7.52±1.07)mmol/L and(0.87±0.16)mg/ml of the control group,and the difference was statistically significant(P<0.05).Conclusion Compound Huangqi Decoction combined with calcitriol has a significant therapeutic effect on patients with diabetic nephropathy,which can reduce blood glucose levels and improve renal function.It is worthy of popularization and application.
