摘 要:目的对比呼吸道深部真菌感染时不同检测方法的结果 ,以此探索最佳的检测方法。方法 317份疑为深部真菌感染患者的下呼吸道标本,同时用真菌培养法、10%KOH湿片法、革兰染色法和荧光染色法4种方法检测,对比真菌培养法、10%KOH湿片法、革兰染色法和荧光染色法的检测结果 ,分别比较其检测的阳性率。结果荧光染色法检测阳性率为89.3%,革兰染色法检测阳性率为76.0%,真菌培养法检测阳性率为73.8%, 10%KOH湿片法检测阳性率为63.4%。荧光染色法检测阳性率高于革兰染色法、真菌培养法、10%KOH湿片法,差异有统计学意义(P<0.05)。革兰染色法检测阳性率与真菌培养法比较,差异无统计学意义(P>0.05)。革兰染色法、真菌培养法检测阳性率均高于10%KOH湿片法,差异有统计学意义(P<0.05)。结论荧光染色是一种快速有效的呼吸道标本的真菌检测方法 ,联合真菌培养可提高呼吸道标本真菌检出率,更好的辅助临床诊断和治疗。Objective To compare the results of different detection methods for deep-seated fungal infection of respiratory tract and explore the best detection methods. Methods A total of 317 lower respiratory tract specimens suspected of deep-seated fungal infection were detected by fungal culture method, 10% KOH wet film method, Gram staining method and fluorescent staining method. The detection results of fungal culture method, 10% KOH wet film method, Gram staining method and fluorescent staining method were compared, and the positive rate of detection was compared. Results The positive rate of fluorescence staining was 89.3%, the positive rate of Gram staining was 76.0%, the positive rate of fungal culture was 73.8%, and the positive rate of 10% KOH wet film method was 63.4%. The positive rate of fluorescence staining method was higher than that of Gram staining method, fungal culture method, and 10% KOH wet film method, and the difference was statistically significant(P<0.05). There was no statistically significant difference in the positive rate of Gram staining method and fungal culture method(P>0.05). The positive rate of Gram staining method and fungal culture method was higher than that of 10% KOH wet film method, and the difference was statistically significant(P<0.05). Conclusion Fluorescence staining is a fast and effective method for detecting fungi in respiratory tract specimens. Combined with fungal culture, it can increase the detection rate of fungi in respiratory tract specimens and better assist clinical diagnosis and treatment.
