新型冠状病毒肺炎胸部CT和病毒核酸检测早期诊断效能分析

摘　　要：目的:探讨新型冠状病毒肺炎(COVID-19)胸部CT表现和病毒核酸检测的早期诊断效能,通过分析给出更优的早期诊断方案。方法:回顾性分析台州恩泽医疗中心2020年1月至2020年2月收治的51例新型冠状病毒肺炎病例,运用R语言统计软件(R3.5.2)分析首次胸部CT检查和核酸检测结果。结果:首次胸部CT检查典型者36例,不典型者15例(其中1例胸部CT表现无异常);首次病毒核酸检测阳性36例,阴性15例;单用CT诊断敏感性0.71,单用病毒核酸检测敏感性0.71,两者差异运用McNemar’s Chi-squaredtest诊断效能相似(P=1),两者联合敏感性达0.94。结论:胸部CT表现和病毒核酸检测在新型冠状病毒肺炎的早期诊断中,诊断效能相似,两者联合可明显提高对新型冠状病毒肺炎的诊断效能。Purpose:To investigate the early diagnosis performance of Novel Coronavirus Pneumonia(COVID-19)by chest CT and viral nucleic acid detection and to find a good early diagnosis method.Method:Fifty-one patients admitted by Enze Medical Center in Taizhou were included for retrospective research.R language was used for statistical analysis.McNemar’s Chi-squared test was used to assess the first plain chest CT images and the first nucleic acid detection results of patients,and the efficacy and clinical significance of these two methods for early diagnosis of COVID-19 were compared.Results:Twenty-nine men and 22 women were included.The sensitivity of CT diagnosis was 0.71 while viral nucleic acid detection was 0.71.There was no statistical difference between the above two methods according to McNemar’s Chi-squared test(P=1)and the combined sensitivity was 0.94.Conclusion:Chest CT and viral nucleic acid detection perform similarly in the early diagnosis of COVID-19,and the combination of the two methods can significantly improve the diagnosis of COVID-19.
