摘 要:目的:基于Meta分析,系统评价血流限制训练对肌骨疼痛程度的干预效果。方法:检索PubMed、EBSCO、Science Direct、Cochrane Library、Google Scholar、中国知网、万方和维普数据库等数据库中关于血流限制训练对肌骨疼痛干预的随机对照试验(RCT),检索时限从数据库建库至2020年3月。根据纳入和排除标准筛选文献、提取资料并且评价纳入研究的方法学质量后,采用Comprehensive Meta-analysis 3.3软件进行Meta分析。结果:共纳入10项RCT,共397例患者。Meta分析结果显示:与对照组相比,长期的BFR训练在肌骨疼痛康复的效果存在显著性优势(Hedge’s g=0.729,95%CI:0.242~1.152,Z=1.217,P=0.003),但短期训练优越性不显著(Hedge’s g=0.504,95%CI:-0.145~1.152,Z=1.522,P=0.128)。进一步的亚组分析和Meta回归分析显示,短期的BFR训练显著优于高强度抗阻训练(Hedge’s g=1.191,95%CI:0.346~2.036,Z=2.763,P=0.006),且短期BFR训练加速肌骨疼痛的康复效果随着患者年龄的增加表现出下降趋势。结论:基于研究间的高异质性,谨慎推荐长期的BFR训练用于提升肌骨疼痛康复效果,40周岁以下成年人可采取短期的规律性BFR训练。Purpose:Based on meta-analysis,the effectiveness of blood flow restriction training(BFRT)on musculoskeletal pain rehabilitation was investigated.Methods:The randomized controlled trials on BFRT for pain were retrieved from PubMed,EBSCO,Science Direct,Cochrane Library,Google Scholar,CNKI,WANFANG Date,Chongqing VIP until March,2020.The inclusion and exclusion criteria were used to select literature,extract data and evaluate the methodological quality of included studies.Meta analyses were performed by using Comprehensive Meta-analysis 3.3 software.Results:Ten RCTs were included,involving 397 participants.Meta-analysis results showed that:compared with control group,long-term BFR training has more advantage in musculoskeletal pain rehabilitation(Hedge’s g=0.729,95%CI:0.242~1.152,Z=1.217,P=0.003).Further subgroup analysis and meta-regression analysis showed that short-term BFR traing is superior to high intensity resistance training(Hedge’s g=1.191,95%CI:0.346~2.036,Z=2.763,P=0.006),and the effect of short-term BFR training on the recovery of musculoskeletal pain decreased with the increase of patients’age.Conclusion:Based on the high heterogeneity between studies,long-term BFR training is cautiously recommended to improve the rehabilitation effect of musculoskeletal pain,and adults under 40 years old can choose short-term regular BFR training.
