摘 要:目的:探索外周循环血环状RNA730(circRNA730)对尿源性脓毒症早期诊断及预后评估的价值。方法:选择2019年1~11月同济大学附属同济医院泌尿外科及外科重症监护病房(SICU)尿源性脓毒症患者40例,取外周循环血并提取血总RNA,收集患者临床资料并随访患者预后。同时选取健康对照人群40例,取其外周血并提取血总RNA,使用实时荧光定量PCR(RT-qPCR)比较两组人群外周血circRNA730相对表达情况并用ΔΔCT法进行分析。绘制受试者工作曲线(ROC曲线)判断circRNA730对尿源性脓毒症的诊断效能,随访患者28 d,记录患者预后情况,比较生存组与死亡组外周血circRNA730的相对表达情况并分析circRNA730对患者预后判断的意义。结果:尿源性脓毒症组与健康对照组患者外周血circRNA730表达存在明显差异(P<0.001),受试者工作曲线(ROC曲线)提示circRNA730对尿源性脓毒症的诊断有较好的价值,ROC曲线下面积AUC为0.827。死亡组外周血circRNA730相对表达量较生存组较高,但两组circRNA730表达情况无明显差异(P=0.547)。结论:外周血circRNA730可以作为尿源性脓毒症早期诊断的生物标准物。Objective: To study the value of peripheral circRNA730 in the early diagnosis and prognosis evaluation of urosepsis. Method: From January 2019 to November 2019, 40 urosepsis patients in the urology and surgical intensive care unit(SICU) of Tongji Hospital of Tongji University were selected as experiment group, while 40 cases of healthy people were regarded as normal group. The relative expression of the peripheral blood circRNA730 was compared by the real-time quantitative PCR(RT-qPCR) and analyzed by ΔΔCT method. Then we recorded the survival time and the expression of circRNA730 during follow-up period of 28 days. The receiver operating characteristic curve(ROC curve) was drawn to determine the diagnostic efficacy of the circRNA730 on the urosepsis. Result: There was a significant difference in the expression of circRNA730 between the urosepsis group and healthy control group(P<0.001). Meanwhile, the subject’s working curve(ROC curve) suggested that the circRNA730 had a good value for the diagnosis of urosepsis, and the AUC of the ROC curve was 0.827. The relative expression of circRNA730 in the peripheral blood of the death patients of urosepsis group were high, but there was no significant difference compared with the survival patients of the urosepsis group(P=0.547). Conclusion: The peripheral blood circRNA730 can be used as an effective biomarker for the early diagnosis of urosepsis.
