盐酸罗哌卡因联合舒芬太尼硬膜外麻醉用于产妇分娩镇痛的临床效果

摘　　要：目的观察盐酸罗哌卡因联合舒芬太尼硬膜外麻醉用于产妇分娩镇痛的临床效果。方法选取2018年3月-2019年3月江西省景德镇市妇幼保健院收治的行无痛分娩的产妇60例,采用信封法随机分为观察组(n=30)和对照组(n=30)。对照组给予盐酸罗哌卡因硬膜外麻醉,观察组给予盐酸罗哌卡因联合舒芬太尼硬膜外麻醉。比较2组镇痛指标、产程时间、不同时间点疼痛情况及分娩方式。结果观察组镇痛起效时间、镇痛完全阻滞时间、第一产程、第二产程及第三产程时间均短于对照组(P <0.01),镇痛持续时间长于对照组(P <0.01);麻醉后10 min、1 h、2 h,2组VAS评分均低于麻醉前,且观察组低于对照组(P <0.05);观察组自然分娩率高于对照组(P <0.05),剖宫产率低于对照组(P <0.05)。结论盐酸罗哌卡因联合舒芬太尼用于产妇分娩镇痛的效果较好,可缩短产程时间,有效改善分娩结局,具有临床推广应用价值。Objective To observe the clinical effect of ropivacaine hydrochloride combined with sufentanil epidural anesthesia for analgesia during labor. Methods From March 2018 to March 2019,60 cases of puerperae who gave birth without pain were admitted to the Jingdezhen Maternal and Child Health Hospital in Jiangxi Province. Using envelope method,they were randomly divided into observation group( n = 30) and control group( n = 30). The analgesic indexes,labor time,pain at different time points and delivery methods were compared between the two groups. Results The analgesic onset time,the analgesic complete block time,the first,second,and third stages of labor in the observation group were shorter than those in the control group( P < 0. 01),and the duration of analgesia was longer than that in the control group( P < 0. 01);10 min,1 h,2 h after anesthesia,the VAS scores of the two groups were lower than before anesthesia,and the observation group was lower than the control group( P < 0. 05);The natural delivery rate of the observation group was higher than the control group( P <0. 05),the cesarean section rate was lower than the control group( P <0. 05). Conclusion Ropivacaine hydrochloride combined with sufentanil has a good effect on maternal labor analgesia,can shorten the labor time,effectively improve the delivery outcome,and has clinical application value.
