带状疱疹运动性麻痹28例临床分析

摘　　要：目的探讨带状疱疹运动性麻痹的临床特征,提高对本病的认识。方法回顾性分析1287例带状疱疹患者中发生运动性麻痹的28例患者的临床特征,复习相关文献并进行讨论。结果1287例住院带状疱疹患者发生运动性麻痹28例(2.18%),男16例,女12例,年龄28-85岁,平均年龄(58.03±5.43)岁。皮损分布部位:头面部21例,肩背部及上肢4例,腰骶部及下肢3例;运动性麻痹:面瘫19例,面瘫及声带麻痹2例,上肢无力4例,下肢无力2列,排尿及排便困难1例。经抗病毒、糖皮质激素、营养神经、止痛、局部物理治疗及后期康复治疗,随访半年,24例(85.71%)患者功能完全恢复。结论带状疱疹运动性麻痹以中老年为主,男女发病率相似。病程长,以各种运动神经受累为特征。早期经过正规治疗愈后良好。肌电图及磁共振成像(MRI)可作为辅助诊断和评估病情的方法。Objective To investigate the clinical features of herpes zoster complicated with motor paralysis.Methods The clinical features of 28 cases of herpes zoster complicated with motor paralysis were analyzed,and related literatures were reviewed and discussed.Results For the 28 herpes zoster patients complicated with motor paralysis,16 were males and 12 were females,age ranged from 28 to 85 years,and the average age was(58.03±5.43)years.The distribution of skin lesions:head and face in 21 cases,shoulder,back and upper limbs in 4 cases,lumbosacral and lower limbs in 3 cases.There were 19 cases of facial paralysis,2 cases of facial paralysis complicated with vocal cord paralysis,6 cases of limb weakness,and 1 case of uroschesis complicated with dyschezia.The 28 patients were treated with antiviral drugs,glucocorticoid,neurotrophic drugs,analgesia,local physiotherapy and late rehabilitation.After 6 months of follow-up,24 patients(85.71%)recovered completely.Conclusions Herpes zoster complicated with with motor paralysis is mainly found in the elderly,and the morbidity of male and female is similar,which has a long course,and characterized by various motor nerve damaged.The disease is usually cured well after early standardized treatment.Electromyography and MRI can be used as important means of diagnosis and assessment of the disease.
