三氧水疗联合托毒生肌汤治疗糖尿病皮肤溃疡的临床疗效研究

摘　　要：目的分析三氧水疗联合托毒生肌汤治疗糖尿病皮肤溃疡的临床疗效,探讨一套三氧水疗联合托毒生肌汤的标准换药方案,结合湿性创面愈合理论解决中药长时间在创面停留的问题。方法于2016年6月—2018年6月在全院范围内收集符合入组条件的患者入组并随机分组,实验组30例患者执行三氧水疗联合托毒生肌汤的标准换药方案,对照组30例患者采取一般换药方案,统一使用碘伏清洗,凡士林纱块外敷换药。比较两组患者的创面愈合速度(创面面积除以创面愈合时间)、平均住院时间和住院费用。结果实验组创面愈合速度、平均住院时间和住院费用(0.51±0.13)cm^3/d、(20.21±4.56)d、(2.39±0.44)万元均优于对照组(0.24±0.01)cm^3/d、(25.56±5.21)d、(3.05±1.22)万元,差异有统计学意义(t=8.021、8.845、7.921,P<0.05)。结论三氧水疗联合托毒生肌汤治疗糖尿病皮肤溃疡的临床疗效确切,可加速创面愈合,缩短住院时间,减少医疗费用。Objective To analyze the clinical efficacy of the treatment of diabetic skin ulcers with the combination of three oxygen hydrotherapy and Tuodu Shengji decoction,and to explore a standard dressing change plan of the combination of three oxygen hydrotherapy and Tuodu Shengji decoction,and to solve the long-term use of traditional Chinese medicine with the theory of moist wound healing problem of staying on the wound.Methods The patients who met the enrollment conditions were collected from the whole hospital and randomly grouped from June 2016 to June 2018.30 patients in the experimental group were treated with a standard dressing change plan of three oxygen hydrotherapy combined with Tuodu Shengji Decoction,and 30 patients in the control group were treated with general dressing change in the program,uniform use of iodophor for cleaning,and topical dressing of petrolatum gauze.The wound healing speed(wound area divided by wound healing time),average length of stay and hospitalization expenses were compared between the two groups.Results The wound healing speed of the experimental group,average hospitalization time and hospitalization expenses were(0.51±0.13)cm3/d,(20.21±4.56)d,(2.39±0.44)million yuan,all better than the control group's(0.24±0.01)cm3/d,(25.56±5.21)d,(3.05±1.22)million yuan,the difference was statistically significant(t=8.021,8.845,7.921,P<0.05).Conclusion The clinical effect of three oxygen hydrotherapy combined with Tuodu Shengji Decoction in the treatment of diabetic skin ulcers is definite,which can accelerate wound healing,shorten hospitalization time,and reduce medical expenses.
