摘 要:目的探讨早期肠内营养支持在糖尿病并发脑卒中昏迷患者中的应用效果及其安全性。方法选取2018年5月—2019年12月入住该院的糖尿病并发脑卒中昏迷患者80例,根据营养支持方法分为观察组和对照组,每组40例。观察组患者发病后24 h内给予肠内营养支持,对照组患者发病后24 h内给予全静脉营养支持,比较两组的营养状况及血糖水平、机体免疫水平、不良反应发生率。结果观察组患者营养支持后的PA、ALB、TP水平均有显著提高,且高于对照组,差异有统计学意义(P<0.05);且干预后的血糖水平,显著低于对照组,差异有统计学意义(P<0.05)。观察组患者营养支持后的IgG、IgA、IgM水平均有显著提高,且高于对照组,差异有统计学意义(P<0.05)。观察组患者的腹泻、便秘、上消化道出血、电解质紊乱等不良反应发生率5.0%,显著低于对照组25.0%,差异有统计学意义(P<0.05)。结论对糖尿病并发脑卒中昏迷患者在早期实施肠内营养支持,可显著改善患者的机体营养状况,提高患者的机体免疫力,且不良反应较少,安全性更高。Objective To investigate the effect and safety of early enteral nutrition support in diabetic patients with stroke and coma.Methods A total of 80 diabetic patients with stroke and coma who were admitted to the hospital from May 2018 to December 2019 were selected and divided into observation group and control group according to nutritional support methods,with 40 cases in each group.Patients in the observation group were given enteral nutrition support within 24 h after the onset of the disease,and patients in the control group were given total intravenous nutrition support within 24 hours after the onset of the disease.Compared the nutritional status,blood sugar level,immune level,and incidence of adverse reactions between the two groups.Results The levels of PA,ALB,and TP in the observation group were significantly increased after nutritional support,and were higher than those in the control group,the difference was statistically significant(P<0.05);and the blood glucose level after intervention was significantly lower than in the control group,the difference was statistically significant(P<0.05).The levels of IgG,IgA and IgM in the observation group were significantly increased after nutritional support,and were higher than those in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions such as diarrhea,constipation,upper gastrointestinal bleeding,and electrolyte imbalance in the observation group was 5.0%,which was significantly lower than 25.0%in the control group,the difference was statistically significant(P<0.05).Conclusion Early enteral nutrition support for patients with diabetes complicated with stroke and coma can significantly improve the nutritional status of the patient's body,improve the patient's body immunity,and have fewer adverse reactions and higher safety.
