非体外循环冠状动脉旁路移植术后新发心房颤动与心脏结构及功能的相关性分析

摘　　要：目的探讨非体外循环冠状动脉旁路移植术后新发心房颤动(房颤)与心脏结构及功能的相关性。方法回顾性分析2017年3月至2019年8月在武汉亚洲心脏病医院行非体外循环冠状动脉旁路移植术的230例患者的临床资料,将其中术后新发房颤的120例患者作为房颤组,无新发房颤的110例患者作为非房颤组。采用单因素分析和多因素logistic回归法分析心脏结构及功能和术前影响手术的相关因素与术后新发房颤发生的相关性。结果单因素分析显示,房颤组患者的收缩期左房内径、左室舒张末期容积、体重指数、收缩压、舒张压、左室射血分数、合并二尖瓣关闭不全比例与非房颤组比较差异有显著性(P<0.05)。多因素logistic回归分析显示,收缩期左房内径(OR=1.226,95%CI 1.105~1.361,P=0.0001)、体重指数(OR=2.537,95%CI 1.350~4.769,P=0.0038)和舒张压(OR=1.061,95%CI 1.022~1.101,P=0.0019)是非体外循环冠状动脉旁路移植术后新发房颤的独立危险因素。结论收缩期左房内径、体重指数和舒张压是非体外循环冠状动脉旁路移植术后新发房颤的独立危险因素。Objective To investigate the correlation between new atrial fibrillation after off-pump coronary artery bypass grafting and cardiac structure and function.Method The clinical data of 230 patients who underwent off-pump coronary artery bypass graft surgery at Wuhan Asia Heart Hospital from March 2017 to August 2019 were retrospectively analyzed.Among them,120 patients with postoperative atrial fibrillation were taken as In the atrial fibrillation group,110 patients who did not develop atrial fibrillation were regarded as the non-atrial fibrillation group.Univariate analysis and multivariate logistic regression were used to analyze the correlation between cardiac structure and function and related factors affecting surgery and the occurrence of new postoperative atrial fibrillation.Result Univariate analysis showed that the ratio of LAD,LVEDV,BMI,systolic blood pressure,diastolic blood pressure,LVEF,and MI in the AF group was significantly different from that in the non-AF group(P<0.05).Multivariate logistic regression analysis showed that LAD(OR=1.226,95%CI 1.105-1.361,P=0.0001),BMI(OR=2.537,95%CI 1.350-4.769,P=0.0038)and diastolic blood pressure(OR=1.061,95%CI 1.022-1.101,P=0.0019)pressure is an independent risk factor for new atrial fibrillation after off-pump coronary artery bypass grafting.Conclusion LAD,BMI and diastolic blood pressure are independent risk factors for new atrial fibrillation after off-pump coronary artery bypass grafting.
