静脉腔内激光消融配合术后间歇充气加压治疗大隐静脉曲张的疗效分析

摘　　要：目的探讨静脉腔内激光消融配合术后间歇充气加压治疗大隐静脉曲张的临床效果。方法选取2017年4月至2019年3月山西省人民医院收治的320例大隐静脉曲张患者,采用随机数字表法分为观察组和对照组,每组160例。对照组采用静脉腔内激光消融术治疗,观察组采用静脉腔内激光消融配合术后间歇充气加压治疗,比较两组患者的手术时间、切口数量、切口长度、术中出血量、术后住院时间、患肢静脉血流速度、凝血功能以及术后并发症(下肢深静脉血栓、皮下淤血、皮下硬结、皮肤麻木感和切口感染)的发生情况。结果两组手术时间、切口数量、切口长度、术中出血量比较差异无显著性(P>0.05)。观察组术后住院时间低于对照组,术后患肢髂外静脉、股静脉、腘静脉平均血流速度高于对照组,凝血酶原时间、活化部分凝血活酶时间高于对照组,D-二聚体水平低于对照组,差异均有显著性(P<0.05)。观察组术后并发症发生率为10.0%,明显低于对照组的26.3%,差异有显著性(P<0.05)。结论静脉腔内激光消融配合术后间歇充气加压治疗大隐静脉曲张具有良好的临床效果,住院时间短,并发症发生率低,且能降低血液黏稠度,加快患肢血流速度,值得临床应用。Objective To explore the clinical effect of endovenous laser ablation combined with postoperative intermittent inflation and compression in the treatment of varicose veins.Method A total of 320 patients with varicose veins admitted to Shanxi Provincial People’s Hospital from April 2017 to March 2019 were selected and divided into observation group and control group by random number table method,with 160 cases in each group.The control group was treated with endovenous laser ablation,and the observation group was treated with endovenous laser ablation combined with postoperative intermittent inflation and compression.The operation time,number of incisions,incision length,intraoperative blood loss,postoperative hospital stay,venous blood flow velocity,coagulation function of the affected limb,and postoperative complications(deep vein thrombosis of the lower limbs,subcutaneous congestion,subcutaneous induration,skin numbness and incision infection)occurred.Result There was no significant difference between the two groups in operation time,number of incisions,incision length,and intraoperative blood loss(P>0.05).The postoperative hospital stay of the observation group was lower than that of the control group.The average blood flow velocity of the external iliac vein,femoral vein,and popliteal vein of the affected limb was significantly higher than that of the control group.The prothrombin time and activated partial thromboplastin time were higher than those of the control group.The D-dimer level were lower than those of the control group,and the difference was significant(P<0.05).The postoperative complication rate in the observation group was 10.0%,which was significantly lower than the 26.3%in the control group.The difference was significant(P<0.05).Conclusion Endovenous laser ablation combined with postoperative intermittent inflation and compression for the treatment of varicose veins has a good clinical effect.The hospital stay is short,the incidence of complications is low,and it can reduce blood viscosity an
