摘 要:目的探讨25-羟维生素D、高迁移率族蛋白B1在手足口病患儿表达及临床意义。方法随机选取2017年12月1日-2019年11月1日某院101例HFMD患儿作为观察组,同期选取93例健康小儿作为对照组。统计2组病情程度、生存情况、有无心肌损伤、患者血清25(OH)D、HMGB1,分析各血清指标与病情程度、心肌损伤相关性,绘制受试者曲线分析各血清指标单一及联合诊断HFMD价值。结果观察组血清HMGB1水平高于对照组,25(OH)D水平低于对照组(P<0.05);ROC显示,两者联合诊断HFMD价值优于血清25(OH)D、HMGB1单一诊断,诊断敏感度及特异度分别为68.32%、91.40%;观察组危重型者血清HMGB1水平高于重症型者、普通型者,25(OH)D水平低于重症型者、普通型者(P<0.05);观察组有心肌损伤者血清HMGB1水平高于无心肌损伤者25(OH)D水平低于无心肌损伤者(P<0.05);HFMD患儿病情程度、心肌损伤与血清HMGB1水平(r1=0.713,r2=0.655)呈正相关,与25(OH)D水平(r3=-0.724,r4=-0.702)呈负相关(P<0.05);观察组生存者血清HMGB1水平低于死亡者,25(OH)D水平高于死亡者(P<0.05)。结论25(OH)D、HMGB1在HFMD患儿中呈高表达,与病情程度、心肌损伤、预后有关,有利于临床评估病情指导治疗。Objective To investigate the expression and clinical significance of 25-hydroxy vitamin D[25(OH)D]and high mobility group protein B1(HMGB1)in children with hand-foot-mouth disease(HFMD).Methods Randomly selected 101 children with HFMD in the hospital from December 1 st,2017 to November 1 st,2019 as the observation group,and 93 healthy children were selected as the control group during the same period.Counted the serum 25(OH)D,HMGB1 of the two groups and the observation group with different disease degrees,survival conditions,and with or without myocardial injury,Spearman analyzed the correlation of each serum index with the degree of illness and myocardial injury,and plotted the receiver curve(ROC)to analyze the value of each serum index for single and joint diagnosis of HFMD.Results(1)The serum HMGB1 level of the observation group was higher than that of the control group,and the level of 25(OH)D was lower than that of the control group(P<0.05);(2)ROC showed that the value of the combined diagnosis of HFMD was better than the single diagnosis of serum 25(OH)D and HMGB1,and the diagnostic sensitivity and specificity were 68.32%and 91.40%,respectively;(3)Serum HMGB1 level of critical and severe cases in observation group was higher than that of severe type and normal type,and 25(OH)D level was lower than that of severe type and normal type(P<0.05);(4)Serum HMGB1 levels in the observation group with myocardial injury were higher than those without myocardial injury,and 25(OH)D levels were lower than those without myocardial injury(P<0.05);(5)The degree of illness and myocardial injury in children with HFMD were positively correlated with serum HMGB1 levels(r1=0.713,r2=0.655),and negatively correlated with 25(OH)D levels(r3=-0.724,r4=-0.702)(P<0.05);(6)The serum HMGB1 level of the survivors in the observation group was lower than that of the dead,and the level of 25(OH)D was higher than that of the dead(P<0.05).Conclusions 25(OH)D and HMGB1 were highly expressed in children with HFMD,which was related to the degree
