摘　　要：目的探讨微型岛状皮瓣移植修复手术治疗手指末节外伤性软组织缺损的临床效果。方法选取开平市中心医院2017年8月至2020年1月收治的手指末节外伤性软组织缺损患者76例,按随机数字表法分为2组:对照组38例予以常规皮瓣修复手术治疗,研究组38例予以微型岛状皮瓣移植修复手术治疗。比较2组手术结果(皮瓣颜色异常情况、供区恢复情况、皮瓣温度以及毛细血管反应时间)、治疗效果、治疗前后生活质量(社会功能、躯体疼痛、生理功能及认知功能)评分及术后满意度。结果与对照组比较,研究组皮瓣颜色异常率显著下降,供区恢复良好率显著升高,皮瓣温度显著升高,毛细血管反应时间显著缩短,治疗总有效率、术后生活质量评分、术后满意度均显著升高,差异均有统计学意义(均P<0.05)。结论微型岛状皮瓣移植修复手术治疗手指末节外伤性软组织缺损手术结果理想,能够有效改善供区的恢复情况,同时提高治疗效果,促进手部功能的恢复,提高患者术后的生活质量。Objective To investigate the clinical effect of mini island flap graft on traumatic soft tissue defects of distal-segment finger.Methods Seventy-six patients admitted to Kaiping Central Hospital from August 2017 to January 2020 for traumatic soft tissue defects of distal-segment finger were randomly assigned to receive either conventional flap repair(control group,n=38)or mini island flap graft(study group,n=38).Surgical results(flap color,donor site recovery,flap temperature and capillary response time),therapeutic efficacy,quality of life(social function,body pain,physical function and cognitive function),and postoperative satisfaction were compared between the two groups.Results Compared with control group,mini island flap graft decreased the rate of abnormal flap color,increased the rate of good donor site recovery,raised flap temperature,and shortened capillary response time(P<0.05).In addition,the total effective rate,scores of quality of life and postoperative satisfaction in study group were higher than those in control group(P<0.05).Conclusion Mini island flap graft can effectively promote the recovery of donor site and hand function,improve the therapeutic effect,and enhance the quality of life in patients with traumatic soft tissue defects of distal-segment finger.
