时空关联成像对健康胎儿房室瓣形态结构的研究

摘  要:目的探讨时空关联成像(STIC)临床应用价值及其成像方法与图像调节技巧。方法应用STIC三维成像技术观察正常胎儿房室瓣立体形态结构,获取97例健康中晚孕期胎儿心脏容积数据,根据初始采集切面的不同分为A、B、C 3组,应用表面成像对胎儿房室瓣进行成像分析,观察胎儿房室瓣立体形态,测量房室瓣环面积及周长。比较各组房室瓣及瓣环的显示率。结果表面成像能立体显示胎儿房室瓣各瓣膜的启闭活动以及房室瓣环的投影形态。从室侧观察房室瓣时A、B 2组中二尖瓣显示率分别为78%(42/54)、52%(11/21),三尖瓣显示率分别为68%(37/54)、43%(9/21);从房侧观察时A、B 2组中二尖瓣显示率分别为65%(35/54)、38%(8/21),三尖瓣显示率分别为59%(32/54)、33%(7/21)。C组容积数据无法完整显示瓣膜形态,显示率为0。A组房室瓣显示率高于B组(P<0.05),从室侧观察房室瓣显示率与从房侧观察差异无统计学意义(P>0.05)。A、B 2组瓣环显示率分别为93%(50/54)、86%(18/21),高于C组(23%),差异有统计学意义(P<0.05),A、B 2组之间瓣环显示率差异无统计学意义(P>0.05)。二、三尖瓣环面积及周长与孕周之间均呈直线相关关系,相关系数r=0.885~0.941,P<0.01。结论STIC三维成像为研究胎儿生理状态下房室瓣膜装置提供一个全新的途径。由于其技术上的局限性,目前仅适于作为科学研究及教学。Objective To assess spatio-temporal image correlation(STIC)applied to observe steric morphological constructions of the normal fetal atrioventricular valves,then further to explore the operative skills in three-dimensional ultrasound imaging and clinically applied value.Methods Ninty-seven cases of normal fetal heart volume datasets were acquired with STIC in the second and third trimester and then were divided into three groups.An offline analysis of the acquired volume was carried out by dedicated 4D software with surface imaging to examine the imaging method and assess the three-dimensional morphology of atrioventricular valve from multiple perspectives.The area and circumference of atrioventricular valve annulus was measured.The statistically significant difference in the imagingrate of atrioventricular valve and valve annulus was compared between groups.Results Besides displaying the steric morphology and activity characteristics of fetal atrioventricular valve,surface imaging can also reveal the projective space of valve annulus.When observing atrioventricular valve from the ventricular side,the imaging rate of mitral valve in group A and B were 78%and 52%,then that of tricuspid valve were 68%and 43%,respectively.Whereas from the atria side,the imaging rate of mitral valve in group A and B were 65%and 38%,the imaging rate of tricuspid valve were 59%and 33%,respectively.The atrioventricular valves in C group were not visualized completely.The imaging rate of atrioventricular valves in group A was higher than which in group B(P<0.05),and it had no obvious distinction when observing on different valve side(P>0.05).The imaging rate of valve ring in group A and B were respectively 93%and 86%,which was much higher than group C(23%)and there was statistically significant difference(P<0.05).The imaging rate of valve ring had no distinct difference between A and B group(P>0.05).There was a linear correlation between the area or circumference of atrioventricular valve annulus and the fetal gestational weeks(r=0.885-0
