运动心电图联合运动负荷超声心动图在冠状动脉严重狭窄病变中的诊断效能评估

摘　　要：目的:探究运动心电图联合运动负荷超声心动图(ESE)在冠状动脉严重狭窄病变中的诊断效能。方法:选取2018年1月~2020年1月就诊于某院的97例疑似冠状动脉严重狭窄患者,进行运动心电图检查和ESE检查,以冠脉造影(CAG)为"金标准",分析两种检查的敏感性、特异性、准确度、阳及阴性预测值等情况。结果:运动心电图联合ESE检查诊断冠脉严重狭窄的特异性、准确度、阳性预测值均高于单纯运用运动心电图及ESE检查(P<0.05),而与单纯采用ESE检查的敏感性与阴性预测值比较无显著差异(P>0.05),与单纯采用运动心电图检查的敏感性与阴性预测值比较差异有统计学意义(P<0.05)。结论:运动心电图及ESE检查诊断冠状动脉严重狭窄病变敏感性均良好,但二者联合使用能提高特异性、准确度及阳性预测值,为无创筛查冠脉病变提供了新思路。Objective:To explore the diagnostic efficacy of exercise electrocardiogram combined with exercise stress echocardiography(ESE)in severe coronary stenosis.Methods:From January 2018 to January 2020,97 patients with suspected severe coronary artery stenosis were selected for exercise electrocardiogram and ESE examination.The sensitivity,specificity,accuracy,positive and negative predictive values of the two examinations were analyzed with coronary angiography(CAG)as the"gold standard".Results:The specificity,accuracy and positive predictive value of exercise ECG combined with ESE in the diagnosis of severe coronary stenosis were higher than those of exercise ECG and ESE alone(P<0.05).Compared with the sensitivity and negative predictive value of ESE examination alone,there was no significant difference(P>0.05),and compared with the sensitivity and negative predictive value of exercise electrocardiogram alone,there was a statistically significant difference(P<0.05).Conclusion:The sensitivity of exercise ECG and ESE is good,but the combination of them can improve the specificity,accuracy and positive predictive value,which provides a new way for noninvasive screening of coronary artery disease.
