关节镜辅助下复位后经皮空心螺钉+钢丝张力带内固定对髌骨粉碎性骨折患者术后膝关节功能的影响

摘　　要：目的:分析关节镜辅助下复位后经皮空心螺钉+钢丝张力带内固定对髌骨粉碎性骨折患者术后膝关节功能的影响。方法:选取某院髌骨粉碎性骨折患者62例(2017年5月~2019年2月),手术方案不同分为切开复位组和关节镜组各31例。切开复位组实施传统切开复位改良钢丝张力带内固定治疗,关节镜组实施关节镜辅助下复位后经皮空心螺钉+钢丝张力带内固定,对比两组围术期基本情况、术后膝关节功能。结果:关节镜组手术用时、术后骨折愈合时间及住院天数较切开复位组短,术中失血量较切开复位组少(P<0.05);关节镜组术后3个月优良率93.55%高于切开复位组70.97%(P<0.05)。结论:关节镜辅助下复位后经皮空心螺钉+钢丝张力带内固定应用于髌骨粉碎性骨折患者,能明显缩短手术用时,加快术后骨折愈合,改善术后膝关节功能。Objective:To analyze the effect of percutaneous cannulated screw+wire tension band internal fixation on knee joint function in patients with comminuted tibiofibular fracture after arthroscopic assisted reduction.Methods:62 patients with comminuted fractures of the tibia in a hospital(May 2017 to February 2019)were selected and divided into two groups according to the surgical plans.The open reduction group(31 cases)was treated with traditional open reduction and modified wire tension band internal fixation.The arthroscopy group(31 cases)underwent arthroscopic assisted reduction and percutaneous cannulated screw+wire tension band internal fixation.The basic condition of perioperative period and the postoperative knee function were compared.Results:The time of surgery,postoperative fracture healing time and length of hospital stay in arthroscopy group were shorter than those in open reduction group,and the blood loss during operation was less than that in open reduction group(P<0.05).The excellent rate of arthroscopy group at 3 months after surgery was 93.55%,which was higher than that of open reduction group(70.97%,P<0.05).Conclusion:Arthroscopically assisted percutaneous cannulated screw+wire tension band internal fixation for comminuted fractures of the tibia can significantly shorten the time of surgery,accelerate postoperative fracture healing and improve postoperative knee function.
