两种静脉脂肪乳剂对早产儿静脉营养相关胆汁淤积的影响

摘　　要：目的:研究两种静脉脂肪乳剂对早产儿静脉营养相关胆汁淤积的影响。方法:通过回顾性病例对照研究,选取某院于2018年4月~2020年2月监护室收治的早产儿140例,将所有早产儿按照随机数字表法划分为对照组(采用豆油静脉脂肪乳剂)与研究组(采用鱼油静脉脂肪乳剂)各70例,观察比较两组早产儿经静脉营养支持后胆汁淤积发生情况。结果:相较于对照组,研究组脂肪乳剂使用时间更短,脂肪乳剂使用总量更少(P<0.05);研究组早产儿的胆汁淤积发生率为11.43%,对照组早产儿的胆汁淤积发生率为25.71%,相较于对照组,研究组患者的胆汁淤积发生率明显更低(P<0.05);相比于对照组,研究组患者的ALT、TBA、AST与y-GT水平更低(P<0.05)。结论:鱼油静脉脂肪乳剂在此次研究中比豆油静脉脂肪乳剂的应用效果更佳,对于减少脂肪乳剂的使用总量,避免胆汁淤积情况出现等有重要意义。Objective:To study the influence of two kinds of intravenous fat emulsion for intravenous nutrition related cholestasis in preterm infants.Methods:140 cases of premature infants treated in a hospital from April 2018 to February 2020 were selected through a retrospective case-control study,and all preterm infants were divided into control group(70 cases were given the intravenous fat emulsion of soybean oil)and study group(70 cases were given the fish oil vein fat emulsion)according to the random number table method.Then,the incidence of cholestasis of two groups of preterm infants were observed and compared after intravenous nutrition support.Results:Compared with the control group,the use time of fat emulsion was shorter and the total amount of fat emulsion was less in the study group(P<0.05).The incidence of cholestasis in the study group was 11.43%,and the incidence of cholestasis in the control group was 25.71%.Compared with the control group,the incidence of cholestasis in the study group was significantly lower(P<0.05).Compared with the control group,the levels of ALT TBA,AST and y-GT in the study group were lower(P<0.05).Conclusion:The application effect of fish oil intravenous fat emulsion is better than that of soybean oil intravenous fat emulsion in this study,and it is great significance to reduce the total amount of fat emulsion and avoid cholestasis.
