摘　　要：目的探讨带操作孔微型腹腔镜单孔法治疗交通性鞘膜积液及腹股沟斜疝患儿的临床效果。方法选取2018年11月至2019年6月于吉安市妇幼保健院儿科接受治疗的50例交通性鞘膜积液及腹股沟斜疝患儿,依据手术方式的不同分为试验组和对照组,每组25例。试验组接受带操作孔微型腹腔镜单孔法治疗,对照组接受带操作孔微型腹腔镜三孔法治疗,比较两组的手术成功率、中转开放手术率、手术相关指标、术后并发症发生率及复发率。结果两组手术成功率及中转开放手术率比较,差异无统计学意义(P>0.05);试验组手术时间、术后下床活动时间及住院时间均短于对照组,差异有统计学意义(P<0.05);试验组术后并发症发生率及复发率均低于对照组,但差异无统计学意义(P>0.05)。结论采用带操作孔微型腹腔镜单孔法治疗交通性鞘膜积液及腹股沟斜疝患儿,手术创伤小,且患儿术后恢复快、复发率低,是一种安全、微创、美观的手术方式。Objective To investigate the clinical effect of mini laparoscopy with operation hole in the treatment of communicating hydrocele and indirect inguinal hernia in children.Methods 50 children with communicating hydrocele and indirect inguinal hernia who were treated in the Department of Pediatrics of Ji'an maternal and child health hospital from November 2018 to June 2019 were selected and divided into experimental group and control group according to different surgical methods,with 25 cases in each group.The experimental group was treated with micro laparoscopic single hole method with operation hole,while the control group was treated with micro laparoscopic three hole method with operation hole.The operation success rate,conversion to open operation rate,operation related indexes,postoperative complication rate and recurrence rate of the two groups were compared.Results There was no significant difference in the success rate and conversion rate between the two groups(P>0.05);the operation time,postoperative ambulation time and hospitalization time in the experimental group were shorter than those in the control group(P<0.05);the incidence of postoperative complications and recurrence rate in the experimental group were lower than those in the control group,but the difference was not statistically significant(P>0.05).Conclusion Mini laparoscopy with operation hole for the treatment of communicating hydrocele and indirect inguinal hernia is a safe,minimally invasive and beautiful surgical method with less trauma,quick recovery and low recurrence rate.
