摘　　要：该院临床药师团队利用“互联网+”信息技术,建立了一种针对2型糖尿病出院患者的新型药学随访服务模式。该模式以医疗机构临床药师团队为主导,以2型糖尿病出院患者合理用药为干预核心。该文系统地总结了该院“互联网+”药学随访的实现途径、促进合理用药的特色优势、模式推广的对策与建议以及注意问题等,为促进医疗机构药师职能转型,药师参与慢病管理提供一定的参考。The clinical pharmacist team of the hospital uses"Internet+"information technology to establish a new type of pharmacy follow-up service model for discharged patients with type 2 diabetes.This model is led by a team of clinical pharmacists in medical institutions,and the rational use of drugs for discharged patients with type 2 diabetes is the core of intervention.This article systematically summarizes the realization of the"Internet+"pharmacy follow-up in the hospital,the characteristic advantages of promoting rational drug use,the countermeasures and suggestions for model promotion,and the attention to problems,etc.,so as to promote the transformation of the function of pharmacists in medical institutions and provide certain reference for pharmacists to participate in chronic disease management.
