阴道彩超与腹部彩超对诊断早期异位妊娠的应用价值

摘　　要：目的比较阴道彩超和腹部彩超在诊断早期异位妊娠中的应用价值。方法选择2016年1月至2018年12月在彩超室行彩超检查后经临床或者病理检查最终确诊的93例早期异位妊娠患者为研究对象,所有患者在彩超室先行腹部彩超检查,再行阴道彩超检查,比较两种彩超检查方法对异位妊娠的检出率。结果腹部彩超组患者检出早期异位妊娠71例,漏诊22例,检出率为76.34%;阴道彩超组患者检出早期异位妊娠89例,疑似2例,漏诊1例,检出率为95.70%。阴道彩超组患者早期异位妊娠的检出率明显高于腹部彩超组。结论与腹部彩超相比,阴道彩超操作简便,安全性高,对早期异位妊娠的诊断更确切,且能够更加准确的显示子宫、附件和盆腔的微小病变。但腹部彩超对子宫肌瘤的诊断优于阴道彩超。在临床检查中将阴道彩超和腹部彩超检查联合应用,不仅能提高异位妊娠的检出率,还能够提高子宫、盆腔和腹腔病变的检出率。Objective To compare the value of vaginal color Doppler ultrasound and abdominal color Doppler ultrasound in the diagnosis of early ectopic pregnancy.Methods From January 2016 to December 2018,93 patients with early ectopic pregnancy who were finally diagnosed by clinical or pathological examination after color Doppler ultrasound examination in the color Doppler room were selected as the research objects.All patients were examined by abdominal color Doppler ultrasound in the color Doppler room first,then by vaginal color Doppler ultrasound.The detection rate of ectopic pregnancy was compared between the two methods.Results Seventy-one cases of early ectopic pregnancy were detected in abdominal color Doppler ultrasound group,22 cases were missed,the detection rate was 76.34%.Eighty-nine cases of early ectopic pregnancy were detected in vaginal color Doppler ultrasound group,2 cases were suspected and 1 case was missed,the detection rate was 95.70%.The detection rate of early ectopic pregnancy in vaginal color Doppler ultrasound group was significantly higher than that in abdominal color Doppler ultrasound group.Conclusion Compared with abdominal color Doppler ultrasonography,vaginal color Doppler ultrasonography is simpler in operation,more accurate in the diagnosis of early ectopic pregnancy,and can more accurately display the subtle lesions of uterus,appendages and pelvis.However,abdominal color Doppler ultrasonography is superior to vaginal color Doppler ultrasonography in the diagnosis of uterine leiomyoma.Therefore,the combination of vaginal color Doppler ultrasonography and abdominal color Doppler ultrasonography in clinical examination can not only improve the detection rate of ectopic pregnancy,but also improve the detection rate of uterine,pelvic and abdominal lesions.
