摘　　要：目的探讨体外循环(CPB)心脏手术患者采取乌司他丁对围手术期心脏炎性反应状态的影响。方法本次研究64例体外循环心脏手术患者均为本院2018年1月至2018年12月接收,随机分为两组,于体外循环心脏手术开始后,对照组(n=32)实施等量生理盐水,研究组(n=32)实施乌司他丁,统计两组不同时间炎性因子水平及心肌酶学指标。结果在单核细胞、WBC及PCT方面,两组T1时刻比较,差异无统计学意义(P>0.05);T2、T3及T4时刻,研究组均较对照组更低,差异有统计学意义(P<0.05);在CTnI及CK-MB方面,T1时刻,两组比较,差异无统计学意义(P>0.05);T2、T3、T4时刻,研究组均较对照组更低,差异均有统计学意义(P<0.05)。结论乌司他丁有助于减轻体外循环心脏手术患者围手术期的炎性反应,从一定程度上保护心肌。Objective To investigate the effect of ulinastatin on perioperative cardiac inflammatory response in patients undergoing cardiopulmonary bypass(CPB).Methods In this study,64 patients undergoing cardiac surgery with extracorporeal circulation were enrolled in our hospital from January 2018 to December 2018.They were randomly divided into two groups.After cardiopulmonary bypass cardiac surgery started,the control group(n=32)received the same amount of normal saline,while the research group(n=32)received ulinastatin.The levels inflammatory factors and myocardial enzymes in the two growps at different time were counted.Results In terms of monocytes,WBC and PCT,there was no statistically significant difference between the two groups at T1(P>0.05).At the time of T2,T3 and T4,the research group was lower than the control group,and the difference was statistically significant(P<0.05).In terms of CTnI and CK-MB,there was no statistically significant difference between the two groups at T1(P>0.05).At the time of T2,T3 and T4,the research group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion Ulinastatin can reduce inflammation in patients undergoing cardiopulmonary bypass and protect myocardium to some extent.
